Kennisha Rheumatology care and Diagnostics has been around for the last 10 years rendering quality Rheumatology services to more than 13 thousand patients so far.

We were based in the suburbs of Mumbai and then moved to central Mumbai in the end of 2018.

We believe we should now expand our umbrella to include other specialties so that more patients in society with other illnesses also benefit and derive hope and encouragement that they will also be able to lead a healthier tomorrow.

And we are equipped to be able to do so.

With 7 consulting rooms, a fully automated pathology lab, Pharmacy and digital X Ray under the same roof patients do not need to travel to different centers to avail diagnostic services.

Towards this step we must first change our name so as to convey this message to others Doctors and Patients alike.

And hence Kennisha Rheumatology care and Diagnostics now becomes KENNISHA HEALTHCARE.

Bad Obstetric History:
Defined as either
1) 3 recurrent pregnancy losses in the first trimester.

2) Even a single pregnancy loss in the 2nd or 3rd trimester.

3) Premature delivery or death of the child within the first 28 days after delivery.

Causes of such pregnancy losses could be due to

a) Increased tendency of the mothers blood towards clotting which in medicine is known as a prothrombotic state.
The most common condition which can cause this is called Antiphospholipid Antibody syndrome though there are other causes too.

b) Autoimmune Condition in the mother wherein the mothers antibodies prevent the foetus from thriving.

Systemic lupus erythematosus (or SLE) can often do this.
Sometimes SLE can also be associated with Antiphospholipid Antibody syndrome in which case it would be a double whammy.

Sjogrens Syndrome is another Autoimmune disease which can be responsible.

A detailed history and clinical evaluation followed by appropriate investigations into the cause of the miscarriage are imperative.

Whilst these definitions and criteria are based on clinical evidence these have been laid down some years ago.

In today's day and age should we wait for 3 miscarriages to occur?
Marriage alliances now occur at a later age with women being as educated as men and hence even a single pregnancy loss is a precious one.

Time will tell...

Scleroderma a dreadful disease and needs timely attention.

Once the damage has set in, it becomes irreversible.

The digital ulcers at the finger tips indicate that the arteries at the tips are narrowed now.

Breathlessness is most often a sign of lung fibrosis or high pressure in the pulmonary arteries.
Referred to as pulmonary arterial hypertension ( PAH).

Once these set in the gradual downhill slope has begun.

Patients do not understand the signs till the disease has advanced.

Who will teach them ?

In an ideal world it would be upto the doctors who do not have the time to explain all this. That too if the patient knows who to approach.

Hence the role of patient support groups is crucial. Headed by a few coordinators who train with a rheumatologist and then in turn teach patients through awareness campaigns, websites, banners, helpline etc.

India now has a scleroderma support group as well.

By patients for the patients.

It is called Scleroderma India.

Indian Patients with scleroderma can now all get together and form a cohesive group and help each other.

Together we can move mountains…

How to visit the clinic with travel restrictions in place?
After you have booked an appointment a sms will be sent to you with the appointment time & details. This sms along with the patient file can be shown to the authorities concerned and you will be allowed to travel to the clinic.

Utmost care is taken to keep each one of you safe, the clinic is sanitized after each patient and only 1 relative is allowed per patient.

Wearing a mask at all times is compulsory.

Dermatomyositis.
(Dermato = related to skin,
Myo= muscle,
Itis=inflammation.)
Refers to inflammation in the muscle and skin.
A lesser talked about auto immune disease compared to its more illustrious cousins like SLE or Sjogrens syndrome.
Our entire body functions because of our muscles.
When we walk or carry any object we use our muscles.
When we breathe we use our respiratory muscles.

When we speak we use our mouth and tongue - muscles.

When we eat we use our stomach and intestines- muscles.

So when an autoimmune disease affects our muscles can we even survive?

It can start suddenly or slowly with muscle aches and pains, joint pains or just generalized weakness.
And this is followed by inability to do things we did easily earlier like climbing stairs or getting from the floor.
There may be a reddish rash over the face and skin when it is called dermatomyositis or without when it is called polymyositis (poly = multiple, myo = muscle, itis = inflammation). The cause is autoimmune and in middle aged ot elderly patients it may even be a sign of malignancy.

And we got Vaccinated too.
Our staff was worried.
They were relying on the encyclopedia of whatsApp which had all but declared that the vaccine was as bad as covid infection itself.
Their families wouldn't permit them to take the jab.

So the 4 of us decided to lead by example.

We went on the 1st of Feb and got the first shot.
Sure my sister got bad chills and fever for a day and my wife had sever bodyache.
But these symptoms were transient and they were on their feet the next day.

I got fever 48 hours later just when I was thinking that maybe the vaccine didn't work on me 😅.

Our staff kept watching us warily for a few days and when they realized that we were still alive and working, they gradually summed up the courage to take the first dose of the vaccine.
Now they know that they will live on and may be in a safer world too.

The Government has done a marvelous job.
The vaccine centers are s**c and span, staff is polite and the process is so well coordinated. Simply amazing. Kudos. 👏

Psoriatic Arthritis

Quite literally stands for Psoriasis and arthritis.
So you have a double whammy here.

Inflammation of the skin and Inflammation on the joints.

To add to this are the multiple variables this combination brings forth.
There are many kinds of Psoriasis and 5 kinds of arthritis under the umbrella of the label Psoriatic arthritis.

So you can have arthritis restricted to the terminal finger joints, a rheumatoid like Psoriatic arthritis, a spinal arthritis similar to Ankylosing spondylitis, a destructive kind of arthritis called arthritis mutilans.

As if this wasn't enough the social stigma of a skin rash over the face, forehead, hands makes it so difficult for the patient.

And the final nail on the coffin is the internal affliction that this disease is so often known to be associated with including Diabetes, hypertension, fatty liver and so on.

Fibromyalgia

A Chronically painful condition that is not visible to others, wherein a perfectly normal looking patient is actually truly suffering from pain, fatigue, lack of sleep and lethargy.

The mind is desirous but the body is unwilling to move.

Routine daily activities that all of us take for granted are also an effort.

Pain amplifies over time and makes one disabled.

Chronic pain results in memory loss, poor concentration power, depression and is often associated with irritable bowel symptoms and can even be associated with hormonal disturbances.

The pain syndrome amplifies over time and is nowadays referred to as chronic wide spread pain so as to encompass all the different kinds of pain that may not typically fit into the dictionary definition of Fibromyalgia.

Without doubt! It's Gout.
Sudden excruciating pain in the middle of the night or early morning waking up one from sleep and rising to a crescendo where even the touch of a bed sheet makes him scream in pain.
He would rather not go to the loo because of the unbearable pain that would occur on weight bearing.

On appropriate medication the pain and swelling subside within 48 to 72 hours and the person looks normal as if this never happened.

Only to recur periodically.

Common sites are the great toe, foot and ankles.

Over time other joints can be involved if the problem becomes chronic.

Can also cause kidney stones.

Males mostly.
Women are blessed till menopause at least.
As are children.
Thank God for small mercies.
This was once called the rich man's disease because triggers are excess of alcohol, red meat.
Diabetes and hypertension ( high BP) and high Cholesterol levels are associations.
Dehydration and certain medications are others.
