What is the meaning of Sensitivity?

🔹Sensitivity is the lowest concentration of a disease marker (antigen, antibody, nucleic acid, enzyme, analyte etc.) that can be accurately detected by an instrument - reagent system.

Bilirubin Direct Test
➡️ The principle of bilirubin test is that sulphanilic acid reacts with sodium nitrite to form diazotized sulphanilic acid.

➡️ The principle of bilirubin test is that sulphanilic acid reacts with sodium nitrite to form diazotized sulphanilic acid. Bilirubin reacts with diazotized sulphanilic acid to form azobilirubin (purple colour).

➡️ DMSO catalyzes the formation of azobilirubin from free bilirubin. The intensity of purple colour is measured photometrically at 546nm, which is directly proportional to the concentration of bilirubin.

➡️ Bilirubin is estimated by end point mode.

Multiple myeloma - diagnosis and monitoring-

✅ Total Protein

• Multiple myeloma leads to excess immunoglobulin production.
• Serum total protein is usually elevated due to M-protein spike.

Reference range: 6.2-8.5 g/dL

✅ Albumin

• Albumin helps in staging and prognosis.
• Low albumin is seen in advanced disease and indicates poor prognosis.

Reference range: 3.5–5.0 g/dL

✅ Calcium (Ca²⁺)

• Myeloma causes bone resorption, releasing calcium.
• Hypercalcemia suggests active bone destruction.

Reference range: 8.5 -10.5 mg/dL

✅ Creatinine and Urea (BUN)

• Myeloma proteins can damage the kidneys.
• Elevated levels indicate renal impairment (Myeloma Kidney).

Reference range:

Creatinine: 0.6-1.4 mg/dL
Urea (BUN): 8-23 mg/dL

✅ LDH (Lactate Dehydrogenase)

• General marker of tumor Disease.
• High LDH level correlates with aggressive disease.

Reference range: 225 – 450 IU/L

Osteomalacia/Rickets- diagnosis and monitoring-

✅ Serum Calcium

• Low calcium is a key feature in both osteomalacia and rickets.
• Hypocalcemia indicates impaired bone mineralization or vitamin D deficiency.

Reference Range: 8.5 -10.5 mg/dL

✅ Serum Phosphate

• Phosphate is crucial for bone formation.
• Phosphate levels are often low in rickets due to malabsorption.

Reference Range:

Adults: 3.0-5.0 mg/dL
Children: 4.0-7.0 mg/dL

✅ Alkaline Phosphatase (ALP)

• It is a marker of bone turnover.
• ALP is significantly elevated in both osteomalacia and rickets due to increased osteoblastic activity.

Reference Range: 108 – 306 IU/L

✅ 25-Hydroxy Vitamin D

• It is a best indicator of vitamin D status.
• Low levels confirm vitamin D deficiency, a primary cause of these conditions.

Reference Range: 30-100 ng/mL

✅ Serum Magnesium

• Magnesium is needed for vitamin D metabolism and PTH function.
• Low levels can worsen symptoms and interfere with treatment.

SGOT (AST) Test

➡️ The principle of SGOT (AST) test is that Aspartate transaminase (AST) catalyses the reaction between alpha-ketoglutarate and L-aspartate giving glutamate and oxaloacetate.

➡️ In the presence of malate dehydrogenase (MDH) Oxaloacetate reacts with NADH giving malate and NAD.

➡️ The rate of NADH decrease is determined photometrically at 340nm which is directly proportional to the GOT activity in the sample.

➡️ SGOT is estimated by rate kinetic mode.
