Remember the huge complex ovarian cyst that I posted last?

Operated on that today and it turned out to be mostly endometriotic with chocolate coloured fluid noted clearly draining out. However, unfortunately, frozen section came out to be borderline/low grade papillary tumour.

So we went ahead with a right salpingo-oophorectomy with peritoneal biopsy with infra colic and supra colic omentectomy and we also put in a right ureteric stent under cystoscopic guidance.
The ACOG recommends prioritising fertility preservation in young females with borderline tumours and hence, we preserved the uterus and the left o***y and fallopian tube.
Will update y’all about the final biopsy reports and how she does on follow up.
Post operative,y she is doing fine.

This is the part 2.
No pelvic evaluation is complete without a proper transabdominal evaluation as well.
Points to note here:
1. The thick hyperechoic lines that we are seeing within the cyst are septae and solid component. We can see significant vascularity in the septae.
2. Look at the severe right hydronephrosis, possibly the cyst is right adnexal. It is possibly due to compression by the cyst.
3. Always check liver echogenicity. Here, it is normal.
4. Check for ascitic collection in the dependent parts: in this case the left paracolic gutter. It is free from any fluid collection.

This is Part 1 of an extremely interesting case that we saw today.
History of 7-8 months with severe pain abdomen, young unmarried girl.
Grossly neglected, has never been advised surgery in the past.
Was started on Dienogest in February by a fellow gynaecologist! I was astounded.
CA-125 was 184 in 2925 January. Points more towards mucinous cystadenoma>endometriosis>neoplasia.

This is not the full evaluation, I’m editing the part 2 where I will show you vascularity tomorrow.

Ever wondered what happens AFTER your o**m pickup?

This is a follow-up to the video that I posted one week back about a patient with low AMH who had a miscarriage and then underwent a second cycle of ovarian stimulation with us. We had been able to retrieve two good quality oocyte from her pick up and she has formed a beautiful hatching embryo as you can see in this video.

Keeping our fingers crossed and hoping that she has a positive urine pregnancy test when we transfer this embryo.

Simple. Short. Sweet.
Saves the patient and unnecessary surgery with recurrence rates of less than 5%.

IVF injections simplified.
1. Menotropin: human menopausal gonadotropin—>for follicular growth
2. Ganirelix/Cetrorelix: GnRH antagonist—-> prevents LH surge, prevents ovulation and helps in increasing follicle size.
3. Trigger injection: helps in ovulation
A. HCG trigger
B. Triptorelin trigger (GnRH agonist)
