Hello everyone, myself Dr. Nilesh Tiwari would like to share a case with everyone which I think would be very helpful to all the patients who have issues related to Kidney Stone.

So on 30/06/2022, a patient came to me with a typical symptoms of Renal Calculi which is known as Kidney Stones. After examination we found out that he had 3 stones (4mm, 5mm and 7mm ). Based on the examination the patient was given certain medications and I informed him that the stones will automatically be removed in around 10 to 15 days.

To my surprise the patient called me on 6th day and informed me that the stones were out from his body. The patient couldn't control his excitement and send me the pictures of it.

1. Asymptotic patients should be treated at home isolation.
2. Early detection.
3. RAT, RTPCR, HRCT
4. MONITORING ( BP, Heart Rate, Saturation, temperature)
5. Steroids only when hypoxia, cytokines storm. Not in early stages.
6. HRCT if needed then after 4-5 days.
7. Favipiravir best if started in 72 hours
8. Doxy/ ivermectin not clearly proven but best improvement best in 1st two days
9. NLR & CRP needes to be monitored
10. Ideally choose only one lab so that reports can be comparable.
11. Colchicine in mild to moderate cases.
12. No proning at home, only spirometry and chest physiotherapy
13. Post COVID vaccination (Sputnik/ Covaxin - 1 month later, COVIsheild 3 month later).
14. Mouth/ throat/ nasal hygiene
15. Temperature oral/ spo2 in middle finger
16. 6 minutes walking test very important ( drop of more than 30% from the baseline, or less than 93%)
17. Restricted Oxygen policy in admitted patients.
18. Strict Sugar control specifically in diabetics and patients on Steroids.
19. High degree of suspicion
20. Early or timely referral to CCH or CDH
