Nature is dynamic, so are diseases and body physiology. Being dynamic with time,its likely that in a single child or patient, in a single episode of illness, the treatment may not remain same from beginning throughout the course of disease.
for example, in a case of flu, fever may be there for first two days, nose may be running for two days and cough may start after 3-5 days. so the treatment and its duration, for a patient will be different on different days.
This is just an example and every disease may have different stages and thus there cannot be blanket therapy for any group of symptoms without taking Onset Duration and Progress of symptoms in each case. called as ODP
This is the reason why your doctor would like to have detailed story of how the problem started, with what problem, in what sequence and how the symptom behaved over time till doctor visit.
The more refined information you give, more specific will be the therapy.
Thus if a patient of chronic cough for months or say epilepsy, visits a doctor, you are giving a history that is a recall of months, and likely that it will not be precise.
