51year old, post hysterectomised (TLH+ BSO) ,multigravida with HPR report suggestive of high grade infiltrating endometrial carcinoma
PET Scan suggestive of- solid enhancing nodule in the left iliac fossa adhered to vaginal vault
Plan- Lap excision of vaginal vault lesion with systematic pelvic and retro peritoneal lymphadenectomy.
Intra-op-
There is e/o-
left sided vaginal vault nodular disease
dense adhesions between the vault nodular disease, urinary bladder and the ureterovescical junction
Complete adhesiolysis done,
Retroperitoneum opened.
Bilateral ureters traced upto the ureterovesical junction.
Bilateral parametrectomy done and upper vaginal cuff excised.
Bilateral pelvic lymphadenectomy done.
Systematic retroperitoneal lymphadenectomy done upto the renal hilum.
Omental biopsy taken.
Prophylactic left ureter stenting done.
