This Patient a 45 years female, highly Diabetic having the 1st MTP joint ulcer on foot for 5 years. Various external offloading therapies were tried but failed to heal the ulcer.
The foot looked to have a planter flexon of great toe with pronation of foot and Ankle equinus ( stiffness of Ankle joint)
To correct these deformities we needed to do Modified Jones EHL tenosuspension + PL to PB tendon transfer + Gastroc soleus recession. All three surgeries were done in one sitting.
Posting the Pre Operation and post Operation photos after 1 month and 20 days of surgery.
The ulcer completely healed and should remain healed. We have given her Modified Diabetic foot wears.

This patient 75 years old man already undergone BK ( below knee ) amputation of one leg even after angioplasty of that leg.
Now come with ulcer on other leg with very critical blood flow in foot .Not ready for Peripheral vascular intervention.
Using all the tricks in the books and faith in God, finally was able to give him completely healed wound. Patient now walking with modified footwear on one side and prothetic leg on other side.
