EUS-B-FNA for evaluation of PUO in immunocompromised patient

60 yr old male Post Transplant on Immunosuppressives and Anti platelets. Patient was having PUO since 1 month. PET-CT revealed Large necrotic right Paratracheal lymph node with Increased SUV uptake. In view of risk of bleeding EUS-B-FNA done under IV sedation.

Final Diagnosis - Tubercular Mediastinal Lymphadenopathy (Rif Sensitive)

Unusual cause of Mediastinal Lymphadenopathy
—————————————————————————-

51 year old female had chest pain since 2 months. CT chest showed Mediastinal Lymphadenopathy. She was then referred to Medical oncologist who advised EBUS for further management to rule out malignancy.

We did Bronchoscopy + BAL + Endobronchial biopsy and EUS-B-FNA guided Lymph node biopsy also in same sitting.
Sr. ACE, Calcium was normal. Mantoux test was Negative.

Infective etiology was ruled out in BAL and FNB was suggestive of reticulin rich Non caseating granuloma suggestive of Sarcoidosis.

Final Diagnosis- Stage I Sarcoidosis

Double whammy… 🤔🤔🤔

A 40 year old male presented with Cough and haemoptysis (mild to moderate) since last 2 months

CTPA was done for work up which was suggestive of fibrobronchiectatic lesion. Bronchoscopy was done further to rule out infective etiology. Bronchoscopy showed soft tissue mass lesion in left upper lobe. BAL genxpert was positive and Biopsy was suggestive of very rare tumor, Capillary Haemangioma.

Learning point- 📚
This case highlights the presence of two etiologies at the same time and thus emphasised the fact that even when obvious tumor is visualized still we should always send sample for all infective pathogens as well.

45 year old female on empirical ATT plus steroids for undiagnosed polyserositis came with increasing shortness of breath. Further evaluation confirmed she to be suffering from scleroderma as well based on Positive Autoimmune reports.

PET CT was suggestive of Inflammtory disease

We did Bronchoscopy plus EBUS-TBNA in same sitting to rule out TB/MDR-TB/Reactive Lymphadenopathy

Turned out to be Non small cell lung cancer in EBUS TBNA. BAL ruled out any infective etiology

Final Diagnosis-
NSCLC with Scleroderma

EUS-B-FNA for mediastinal mass with respiratory failure in a patient with ILD
—————————————————
62 year male presented with ILD with respiratory failure and Mediastinal mass. Patient had history of Vasovagal syncope during last attempted CT guided biopsy from mass. As patient was unfit for GA, EUS-B-FNA done under local Anesthesia. Biopsy proved it to be benign cystic lesion.
