🩺 Rare Case Highlight :
Confluent and Reticulated Papillomatosis (CARP) of Gougerot and Carteaud.
🔹 What it is?
A chronic skin disorder characterized by lesions that can start with 1-2 mm papules, first appearing in the intermammary area or less frequently the interscapular region. Lesions rapidly enlarge to 4-5 mm and become brown, keratotic, verrucous papules, patches or thin plaques - "Confluent in the center and Reticulated towards the Periphery"
🔹 Who gets it?
Often seen in adolescents and young adults, but can occur at any age.
🔹 Histopathology 🔬 :
Hyperkeratosis, acanthosis and papillomatosis. Sparse superficial perivascular infiltrates of lymphocytes are also seen. A characteristic feature is the club- shaped, bulbous epidermal rete ridges that protrude slightly into the papillary dermis with pigment at their bases ("dirty feet").
🔹 Dermoscopy 🔍 :
Brownish pigmentation in a reticulated / net-like pattern
Whitish structureless areas or streaks → correspond to papillomatosis / epidermal hyperplasia.
Fine white scaling, sometimes accentuated in skin furrows
Background often appears dirty brown-gray.
🔹 Differential Diagnosis -
Acanthosis Nigricans, Pityriasis Versicolor , Darrier's Disease , Terra firma-forme dermatosis
