A 52 year old male Mr. P, known case of diabetes, obesity, vitiligo. Came with chronic hard core painfull pressure callus present on 5th MT distal to middle phalynx bilaterally.

એક 38 વર્ષીય શ્રીમતી ગુપ્તા, ડાયાબિટીસની પૂર્વ અવસ્થા માં (પ્રી-ડાયાબિટીસ) હતા અને ઉચ્ચ BMI (40 kg/m2) ધરાવતા હતાં. તેમનું વજન 93.6 કિલોગ્રામ હતું, આથી તેમને વજન ઓછું કરવુ હતું. તેમનું Hba1c (ત્રણ મહિનાનું સરેરાશ સુગર) 6.4 હતું, તેઓ શારીરિક રીતે અસક્રિય હતા. તેમના શરીરના બધા માપ માપ્યા. મેં તેમને એક મહિનાની આહાર યોજના (ડાયેટ પ્લેન) આપ્યો. 10 દિવસની અંદર તેણીએ કોઈપણ નબળાઇ વિના 3.2 કિલો વજન ગુમાવ્યું, અમે ખૂબ close ફોલોઅપ્સ કર્યુ, તે નિયમિતપણે તેના રોજિંદા રૂટિન અને વેઇટ ચાર્ટ મોકલે છે. 1 મહિના પછી તેમને 7.4 કિગ્રા ગુમાવ્યા. બીજા 1 મહિના પછી તેણીએ ફરીથી 5.8 કિલો ગુમાવ્યુ. 2 મહિનાના અંતે તે 77.2 કિલોગ્રામ હતા. (ટોટલ તેણીએ 16.4 કિગ્રા ગુમાવી). તેણીએ કોઈ કસરત નહોતી કરી. અને HbA1c-૩ મહિના પછી નું સરેરાશ સુગર 5.2 હતું. આ બધા પ્રયત્નો અને મન નિયંત્રણને કારણે થયું. નવા સ્વસ્થ અને સંતુલિત જીવન માટે ઉત્સાહ.

A 38 year old Mrs Gupta, a prediabetic, wants to lose weight because of high BMI (40 kg/m2). On first day her weight was 93.6 kgs. Vitally normal, HbA1C was 6.4, not physically active. We measured all her measurements. I gave her one month diet plan. Within 10 days she lost 3.2 kgs of weight without any weakness, we did close followups, she regularly send her daily routine and weight chart. After 1 month she lost 7.4 kgs. After another 1 month she again lost 5.8 kgs. At the end of 2 month she was of 77.2 kgs. (Totally she lost 16.4 kgs). She did not do any exercise. And HbA1c- after 3 months it was 5.2. A 73 year old mr. Parmar, a farmer, a diabetic, had non healing ulcer on left foot on great toe since 2 months and had dressing done at local place and not recovered. Outside he was advised for amputation so he Came to opd with worsening of ulcer. Biothesiometry done, I did xray foot which found normal, hba1c is of 13.0 mg/dl. I explained him and his son about the severity, we console him and told them, will recover for sure without amputation"" Long acting insulin has been started along with antibiotics. Offloading explained, We did dressing on every 4th and 5th day. Just after 19 days ulcer got healed, antibiotics gradually stopped, sugars becoming under controlled, he was very happy. After completion of treatment of 27 days he got completely recovered and went back to his village.

એક 73 વર્ષીય શ્રી. પરમાર, ખેડૂત, અનિયંત્રિત ડાયાબિટીસ, 2 મહિનાથી ડાબા પગના અંગૂઠા પર નોન હીલિંગ અલ્સર હતું અને સ્થાનિક જગ્યાએ ડ્રેસિંગ કરતા હતા. એમાં કોઈ રીકવરી આવતી ન હોતી. ત્યાં તેમને અંગવિચ્છેદન (amputation) માટે સલાહ આપવામાં આવી હતી માટે તેઓ અલ્સરની આ સ્થિતિ સાથે ઓપીડી પર આવ્યા. એમનું બાયોથેસિઓમેટ્રી કરવામાં આવ્યું, એક્સરે ફુટ કર્યું જે સામાન્ય લાગ્યું, HbA1c (ત્રણ મહિનાનું સરેરાશ સુગર) 13.0 mg/dl હતું. મેં તેમને અને તેમના પુત્રને સ્થિતિ ની ગંભીરતા અને તિવ્રતા વિશે સમજાવ્યું, તેમને આશ્વાસન આપ્યું અને કહ્યું, "અંગવિચ્છેદન કર્યા વિના રિકવરી પ્રાપ્ત થશે, ચિંતા કરવાની જરૂર નથી, એની બાહેંધરી પણ આપી" એન્ટિબાયોટિક્સની સાથે ઇન્સ્યુલિન શરૂ કરવામાં આવ્યા. Offloading સમજાવ્યુ, અને દર 4 થી 5 માં દિવસે ડ્રેસિંગ કર્યું. ફક્ત 19 દિવસ ના અંતે અલ્સર સારું થઈ ગયા પછી, એન્ટિબાયોટિક્સ ધીમે ધીમે બંધ થઈ ગઈ, સુગર નિયંત્રિત થઈ, તેઓ ખૂબ ખુશ હતા. 27 દિવસની સંપૂર્ણ સારવાર બાદ તેઓ પૂર્ણ રીતે રિકવર થઈ ગયા અને પાછા તેમના ગામ ગયા.
