A 45-year-old male from Kampala, Uganda, with end-stage renal disease (ESRD) on maintenance hemodialysis for 1.5 years and a background of neurogenic bladder, presented in a nutritionally compromised and morbid condition, wheelchair-bound.
Detailed evaluation, including urodynamic studies, revealed a large-capacity bladder with delayed sensation during the filling phase, consistent with an underactive bladder.

The patient and his family were counseled regarding post-transplant bladder management options. Clean intermittent catheterization (CIC) was discussed, including its higher risk of recurrent urinary tract infections and potential adverse impact on graft survival. Ileal conduit urinary diversion was presented as an alternative, with a lower risk of infection and comparable graft outcomes. The patient and family opted for ileal conduit diversion.

A simultaneous kidney transplant and ileal conduit creation were performed on 11 March 2026, with the donor being his sister. The surgery lasted approximately 7 hours.
Postoperatively, the patient had an episode of impaired fluid delivery due to unilateral superior vena cava (SVC) obstruction, which was promptly managed by shifting fluid administration to the opposite-side permacath.

Recovery was uneventful thereafter. Oral intake was initiated on postoperative day 2, and the ileal conduit functioned well from the outset. The patient was discharged on postoperative day 8 with a serum creatinine of 0.9 mg/dL. At discharge, he had improved nutritional intake and had begun ambulating with support.
