When Most of us were giving rest to our thalamus tuning out the stress and preparing for the next day churns .
Yes !! it was at 1 am when Mr Lal Babu Sah was thriving for his limb .

On the behalf of CTVS team headed by Dr Khushwant Popli , I Dr Amrish Kumar feel great pride to save his left leg which was acutely thrombosed . He arrived in great agony with discoloration and an extremely painful leg , which was progressively increasing since 1 week .
His CT Angiography showed acute on chronic completely thrombosed left Popliteal Artery which needed urgent Embolectomy .
With the help of our esteemed MD sir all brisk arrangements were done for an emergency surgery .
We took the patient to OT with our backbone Dr Gaurav jain ( cardiac Anaesthetist) at around 1:30 am and with in less than 30 mins time the successful Embolectomy was done and limb perfusion was restored with bounding peripheral pulses .
During surgery we observed the patient was developing compartment syndrome for which immediately expert opinion was taken from Dr Abhishek sharma(Ortho Team) and decided for fasciotomy . Without a delay Dr Abhishek sharma reached hospital and did a wonderful Fasciotomy.
Now the patient's leg is back with its strength due to an amazing selfless teamwork

I am both honoured and thrilled to share about another successfully done tricky surgery , which we as cardiac surgeons of Aakash hospital feel proud for pushing the boundaries of cardiovascular care .
It was a complicated Redo DVR ( double valve replacement ) surgery of mere 27 yr old mother who underwent DVR in 2019 , Following which her valves got stuck . She was in gross heart failure , significant anemia ,severe PAH and other complications which added on to the risk of mortality .
It was a marathon surgery of 10 hours where not only we successfully replaced the valves taking all challenges including adhesions, pannus present at valves , raised pulmonary pressures but also the post operative period went smooth without any complications and minimal drains . Only after 1 day of surgery she was extubated and on 3rd post op day she was ambulated . It was overwhelming to hear it from patient how happy she is this time undergoing surgery with such good recovery in comparison to the previous surgery .

A PROUD MOMENT- CABG WITH LIMB GIRDLE MUSCULAR DYSTROPHY .
WITH A LIMITED EVIDENCE BASED STUDIES AND TRIALS AVAILABLE THERE IS NO SUCH CASES OF CABG WITH LGMD HAS BEEN DONE OR REPORTED.
Besides striving for clinical excellence we always aspire to do rare cases accepting the challenges associated.
It gives me a immense pleasure on the behalf of CTVS team(Dr Abhay/Amrish) to inform you that we recently did CABG with diagnosis of post PTCA, proximal muscle weakness with limb Girdle disease.

LGMD is a group of genetic conditions that includes various phenotypes with predilection to impairment of shoulder and pelvic girdles and proximal muscles weakness.
Anaesthesia in patient with LGMD are great concern.
Risk are Rhamdomyolysis, malignant hyperthermia unpredictable respiratory depression, prolonged ventilation and tracheostomy.
So under the supervision of our most experienced anaesthetist doctor Gaurav Jain/ Ishan , CABG was performed under total intravenous anaesthesia thoracic epidural and regional block( popliteal sciatic and adductor canal block) in view of his proximal muscle weakness with LGMD
No muscle relaxants /inhalation agents were used during surgery
Patient was extubated on first POD and thoracic epidural catheter was removed on 2nd POD.
