Why is night splinting necessary in severe hand burns?

After a burn, the hand naturally rests in a position of least resistance due to pain, swelling, and tissue tightness.

If not supported at night, this can lead to stiffness, contractures, and long-term deformity.

Night splints help maintain the hand in a functional position while sleeping, preserving range of motion and improving long-term recovery.

Proper positioning today prevents disability tomorrow.

Swelling that doesn’t go away could be more than just water retention.

Lymphedema progresses in stages:
Stage 0 – No visible swelling yet, but damage has begun.
Stage 1 – Soft swelling that may reduce with elevation.
Stage 2 – Firmer, persistent swelling.
Stage 3 – Advanced stage with hard, thickened skin changes.

Early diagnosis can prevent progression. Don’t ignore chronic swelling.

How do we diagnose lymphedema?

Not all swelling is lymphedema. Proper diagnosis requires a structured approach:

1️⃣ Detailed history – surgery, cancer, radiation, infection, duration of swelling
2️⃣ Clinical examination – persistent swelling, skin thickening, Stemmer’s sign
3️⃣ Diagnostic tests – Lymphoscintigraphy and lymphography to assess lymphatic flow

Early and accurate diagnosis prevents progression and long term complications.

Correct evaluation is the first step toward the right treatment.

Burn wound healed? Treatment is not over yet.

After burns, the skin continues to remodel. Excess collagen can lead to thick scars, itching, tightness, and contractures.

That’s why pressure garments are advised.
Made of cotton and lycra, they provide uniform compression that helps:

• Control hypertrophic scarring
• Improve scar maturation
• Reduce itching
• Prevent contractures
• Improve functional recovery

Consistency is the key to better long-term results.

Is your swelling really lymphedema?

Not all swelling is the same. The three most common causes are:

1️⃣ Water retention
2️⃣ Fat accumulation
3️⃣ Lymphatic obstruction (Lymphedema)

Lymphedema happens when protein-rich lymph fluid builds up due to blockage in the lymphatic system, leading to persistent swelling that does not easily reduce.

Correct diagnosis is the first step toward the right treatment.

After a burn heals, the skin often becomes dry, tight, and itchy. This is why coconut oil is commonly recommended during the recovery phase. It helps moisturize the skin, improve elasticity, and contains lauric acid, which has natural antibacterial properties.

Coconut oil should be applied only after the wound has closed, not on fresh or open burns. Proper aftercare plays a key role in better healing and scar quality.

Lymphedema is not just simple swelling. It can be primary, due to developmental abnormalities of the lymphatic system, or secondary, caused by infections, cancer, surgery, or radiation. Understanding the difference is essential for early diagnosis and proper management.

Will a burn leave a scar or heal completely? 🤔
It mainly depends on two things – the depth of the burn and how long it takes to heal. Burns that heal quickly usually don’t scar, but deeper burns taking more than 2–3 weeks often leave marks. Early treatment makes a big difference in recovery.

Not all swelling is just “water retention.”

Lymphedema occurs when the lymphatic system is blocked or damaged. This leads to accumulation of protein rich fluid in the tissues, causing persistent swelling.

The lymphatic system helps in fluid balance, fat absorption, and immune function. When circulation is obstructed, the swelling does not reduce easily and may gradually worsen.

Early recognition and proper management are crucial to prevent long term complications.

Finger drop but wrist can still lift?

If a patient cannot extend the fingers or thumb, yet wrist extension is preserved and there is no sensory loss, think of Posterior Interosseous Nerve (PIN) Syndrome.

It is a pure motor branch of the radial nerve.
That’s why sensation remains intact, but finger extension is lost.

Clinical examination makes all the difference.

Burn pain is unpredictable.

First and second degree burns can be extremely painful because the nerve endings are still intact and exposed.
But in third degree burns, the nerve endings may be completely destroyed — which is why the pain can actually feel less in the burned area.

Severity does not always equal pain intensity.
Understanding burn depth is crucial for proper diagnosis and timely treatment.
