Got a chance to present an interesting case on “ Female Spondyloarthritis “.

Few interesting facts
1) Tarsal tunnel like presentation can happen
2) Females usually present late
3) More peripheral joint involvement like knees ankles
4) Neck involvement is more often than classic inflammatory backache
5) Never fail to revisit diagnosis in case of dilemma
6) Single test never classifies anyone . Its combination of multiple reports and examination along with history ..

When one is dressed up to go back to her Alma mater after 13 years to relive the under graduate days and land up chairing a wonderful session in Rheumatology , that feeling is ineffable.

Had a privelege and an honour to present an interesting case on “ Unusual presentation of Sjogren Syndrome “ in Delhi Rheumatology Update 2025

- Here I presented the case where patient presented to me with CNS symptoms of aseptic encephalitis.
- 3 years down the course , pt presented with typical symptoms of disease like dry eyes , dry mouth and inflammatory polyarthralgia.

Sjogren Syndrome :
- common and easily missed disease
- Sometimes usual manifestations like dry eyes and dry mouth can present later in disease course

Got an opportunity to chair a wonderful session on “ Complementing the compliments “ , basic principle and clinical implications.

Discussed various pathways of compliments involved in AIRDs

- First Drug to be approved for atypical Hemolytic Uremic Syndrome
- Precautions while prescribing eculizumab ( increase risk of encapsulated bacteria- vaccines against strep pneumonia , H. Influenza needed)
- When to discontinue- not before 6 months
- Cost -1.5 lac per vial -30 ml..10mg/mL
- Bring effects within 3 days like normalisation of kidney functions and increasing platelets . -Remission in 7-8days

Privileged to present and share my thoughts on
“ Lab investigations in Rheumatology “

1) Pretest probablity should be high before ordering test .
2) Exact indication should be present
3) Every positive test doesn’t indicate presence of disease
4) Every negative test doesn’t rule out disease
5) ANA positivity doesn’t indicate presence of Lupus
6) RF positivity may be seen in myriad of Rneumatological and non Rheumatological condition

Though a late post but worth mentioning.
- Had an opportunity to present a case on
“ Gut and Spa “, not so simple yet in this meeting we had great discussions on how some specific food items can be boon or bane for some.

- May sound simple but the cobweb of pathogenesis is still not completely understood .

- Happened to present one such case of patient who had joint pains on eating one specific dish.

It’s an absolute pleasure and honour to be a part of this wonderful summit being organised by stalwarts in field of Rheumatology Dr Rohini Handa and Dr SJ Gupta.

# Chaired the session on “ Latest management guidelines for Lupus”

Enthusiastic and fully engrossed audience made it absolutely interactive to keep the spirits high.

# Latest addition being ( from 2019 )
1) Anifrolumab - except for CNS Lupus and Lupus Nephritis
2) Belimumab and Calcineurin inhibitors being placed above Rituximab in Lupus Nephritis management.
3) Cyclophosphamide - Remains corner stone for management of CNS Lupus and Lupus Nephritis

Had an opportunity to chair CME on “ Advancement in autoimmunity diagnosis “.

After crisp lecture by Dr Saurabh ( Lal Path lab ) about various lab tests available, I discussed the clinical aspects of the same .

It was wonderful to interact with various esteemed faculty members and DNB students .
