A 68 year old female came with complaints of non healing ulcer in right foot x 1 year due to thorn prick followed by abscess and debridement done 1 year back on blood investigation patient found to have high blood sugar , increased renal parameter and with low hemoglobin, the wound bed was healthy as the patient was doing regular dressing, then later callus tissues was removed and offload total contact cast given to the patient, Nephrologist opinion sought for ckd and insulin were started and blood sugar level is controlled . after wound healing patient was given Customized foot wear and thought poper regular foot care

A 56 year old patient with T2DM x 8 years came to our hospital with complain of non healing ulcer in the foot x 1 year on treatment with antibiotics . on initial assessment bp-130/90mmhg,hba1c-8.2 ,cbg -281,lipid profile was slightly elevated , crp and other renal parameters were within range. xray was done and no bone changes present. patient was adviced to stop the antibiotics he was taking for past 1 year. patient started on sglt2 inhibitors with additional long acting oha.No advanced wound dressing done,just normal dressing with offloading of the foot done with removable tcc with articast, wound started to heal and size reduced to half in 2 weeks, later patient thought how to do offloading dressing himself with removable tcc and patient was monitored at regular interval. later hba1c-6.4, cbg-150below with smbg monitoring, lipid profile also becames to normal range, wound healed completely. patient was thought diabetes education along good foot care maintenance and given custom made footwear.

A 56 year old male uncontrolled T2DM came with infected abscess on the plantar aspect of left foot followed by little toe amputation 2 weeks ago due gangrene on initial assessment crp elevated ,hba1c -10.2 ,cbg-520 and other blood parameters were normal, tissue and pus sent for culture and sensitivity . Patient was started on iv antibiotics, basal bolus insulin regimen and extensive debridement of the infected tissue was done and customized regular offloading dressing done , cbg was alway below 160 and hba1c reduced to 7 ,the wound started healing . After that patient was thought proper foot care and customized foot wear were given

A 56 year old male. k/c/o uncontrolled t2dm/ckd/b/l charcot foot/anaemia not on regular medication. came with c/o 1.foot ulcer 13x 6cm at back of the ankle from medial to lateral Malleolus x 20 days due to burn injury and 2. foot ulcer 4x4 cm at plantor aspect of leftt foot x 8 months and advised to do below knee amputation of left leg in previous consulation from outside . on initial assessment both ulcer are infected and swab sent for culture and sensitivity, cbg-610, hb-7. patients started on basal bollus insulin Therapy along with iv antibiotics and extensive debridement of the infected ulcer is done . pt had malena and went in to myocardial infarction and patients treated for it along with 5 unit PRBC transfusion done. after that regular wound dressing , with proper offloading and iv antibiotics and strict diabetic control (cbg-below 160) the infection settled and ulcer started healing with primary intension , now both ulcers healed. patient advised to do regular foot care and continue regular medicines.
