December 2025 AYN EYE CARE HOSPITAL 🏥 HYDERABAD Case

An 81-year-old female presents to AYN EYE CARE Hospital with a one-month history of painless vision loss in the right eye. She reported sudden onset of vision loss upon waking without associated pain, redness or trauma. The patient also described seeing colorful lights and reported only being able to see out of a “triangle” of vision.

Acanthamoeba Keratitis
A rare but potentially vision threatening corneal infection

🦠 Cause

🔬 Caused by Acanthamoeba, a free living protozoa
💧 Found in contaminated water, tap water, swimming pools, soil
👓 Strongly associated with poor contact lens hygiene
⚠️ Using contact lenses while swimming or rinsing lenses with tap water increases risk

👀 Signs

🔎 Disproportionate severe pain compared to clinical findings
🌀 Ring shaped stromal infiltrate
🔴 Conjunctival redness
⚪ Perineural infiltrates, a key clinical clue
🧪 Reduced corneal sensation in later stages

🤕 Symptoms

😖 Severe eye pain
🌫️ Blurred vision
💡 Photophobia
😢 Excessive tearing
👁️ Foreign body sensation

🩺 Management

⏱️ Early diagnosis is critical
🧴 Topical anti amoebic agents
• Polyhexamethylene biguanide
• Chlorhexidine
💊 Adjunctive cycloplegics for pain relief
🚫 Steroids only with extreme caution and under specialist supervision
🔁 Long term treatment is often required
🔪 Advanced cases may need therapeutic keratoplasty

⚠️ Key Message

❓Why is this infection often missed
Because it mimics herpes or fungal keratitis in early stages
✔️ Always suspect Acanthamoeba keratitis in contact lens wearers with severe pain

A 13-year-old boy underwent a routine examination. He had no visual symptoms. Wide-field color image showed a large dark-red vascular lesion with a fibrotic component in the temporal fundus, displaying grape-like lesions. Wide-field OCT demonstrated round lesions with hyperreflective borders and either hyporeflective or hyperreflective centers. Fluorescein angiography revealed multiple rounded lesions with a hyperfluorescent upper region and a hypofluorescent lower region, consistent with the “cap sign” which results from the dissociation of erythrocytes and plasma. These findings were in favor of a retinal cavernous hemangioma.

Clinical Snapshot:
80-year-old female with 7–8 years of gradual bilateral vision loss
BCVA: 20/200 OU. Previously advised anti-VEGF injections

Fundus:
Geographic atrophy due to AMD

OCT:
No intra- or subretinal fluid; presence of Outer Retinal Tubulations (ORT)

Image Highlights:
Large circular depigmented patches with prominent choroidal vessels
OCT shows classical ORTs with hypo-reflective cores and hyper-reflective borders, no fluid, and thinned choroid

Key Points:
• ORTs are neurodegenerative changes in the outer retina.
• Seen in AMD, geographic atrophy, choroideremia, mitochondrial diseases.
• They mimic cysts but aren’t fluid—no role for anti-VEGF injections.
• Indicate disorganized outer retina and irreversible photoreceptor loss.
• Marker of poor visual prognosis ⚠️

A Rare Case of Mature Sutural Cataract 🌟

This 62-year-old patient presents with a mature cataract — but look closely and you’ll see the classic Y-shaped sutural opacity at the lens nucleus.

🧬 Sutural cataracts are congenital, often hereditary, and typically non-progressive — but in rare cases like this one, they can gradually worsen and contribute to visual loss later in life.

👁️ Eyelid Alert:
In addition to lens changes, this patient also shows dermatochalasis with lateral hooding — a common age-related change that may contribute to superior visual field defects and ocular surface complaints.

🔍 Key Features:
▪️ Congenital opacity along fetal Y-sutures
▪️ Usually stable, but here it progressed with age
▪️ Mature lens changes now causing visual impairment
▪️ Slit-lamp exam reveals both sutural and mature cataract features
▪️ Dermatochalasis with lateral hooding observed on external exam

💡 Takeaway:
Not all sutural cataracts stay innocent — and don’t forget to assess the lids too! Age-related changes like dermatochalasis can coexist and compound visual complaints in elderly patients.
