He has done National Teacher Training Workshop at MAMC, New Delhi meant for doctors involved in medical graduate and post-graduate teaching. Last but not the least includes his fellowship in Endovascular Neurosurgery at Department of Neurosurgery at Nagoya at Medical University, Japan and advanced courses on Complex Spine Surgery at Kuala Lumpur and AIIMS, New Delhi. He has great accreditations in surgery. Dr. Rahul Gupta has performed surgeries on hundreds of patients with head and spine injury. Tuberculosis is very common in India. It can affect any part of our body. Since, I am a Neurosurgeon. I will tell you about Brain and Spine Tuberculosis . Brain Can have infections such as meningitis or tuberculoma. Spine can have tuberculosis effecting bone, Spinal cord or nerves. We talk about one such patient who developed weakness of lower limb due to tuberculosis. She had severe back pain. was not moving her legs due to tuberculosis her bones were weak & fractured. We did spine surgery to remove pus decompressed the nerves. then, we fixed the spine with screws & rods. So that she can sit & stand. A few days later, she came on wheelchair can walk now. For tuberculosis of brain & spine, Anti- Tubercular Medicines are needed for 18 Months. Sometimes, injections are also needed. If medications are taken in right dosage for required duration this disease can be cured otherwise resistant tuberculosis develops.

Hi I am Dr. Rahul Gupta, I have seen people confusing between a Neurosurgeon, Neurologist and Spine Surgeon. Neurosurgeon does surgery for diseases related to brain, spine and nerves. Neurologist is an physician who treat ailment related to brain, spine or nerves that don't need surgery only medication or physiotherapy is required like chronic brain infection or Parkinson disease, movement disorder, Guillain Barre Syndrome or chronic headache without any tumor or hydrocephalous. Whenever surgery is required, a Neurosurgeon is called immediately. There are many disease which can be treated by both. During MCh training, a Neurosurgeon first learns Neurology then develop surgical skills. If neurosurgeon is not capable of treating difficult neurological cases, they should be referred to a Neuro-physician and vice versa. Similarly, spine surgery can be done by a specialized and trained orthopedic spine surgeon or a Neurosurgeon. A Neurosurgeon is a spine surgeon by virtue of his training but on the contrary an Orthopedic surgeon need special training to become a spine surgeon. Fully trained orthopedic spine surgeon can treat all problems of spine by surgery. Similarly there are certain disease which need procedure in the Neuro-cath lab. There are endovascular procedures where we enter inside the blood vessel to treat vascular problem, similar to cardiac stenting. We can put stent, glue, coils or particles to treat the diseases of blood vassels inside brain. These procedures can be done by a Neurosurgeon Neurologist or Neuro-radiologist who have received special training for the same.
These procedures are tricky & risky & should be performed by person who is competent & confident. These are certain other neurological diseases where patient consults a general physician, gynecologist, endocrinologist or opthalmologist. We have cases where patient visits an ophthalmologist for weeks or months & there is a brain tumor that remains undetected because MRI Brain is not done. It is sometimes mandatory to take second opinion from another specialty. People visit endocrinologist for hormonal problem which can sometimes be due to pituitary tumor, or they visit gynecologist for problem in menstrual cycle which can be due to brain tumor people may visit urology or cardiac surgeon for problem related to Neurosurgery. So, there is a lot of overlap in symptoms & can be treated by many specialties. It is mandatory to refer patient to another specialty for the same.

Hi I am Dr. Rahul Gupta Senior Neurosurgeon at Fortis hospital Noida. It is a common belief that spine surgery leads to paraplegia & inability to walk. This is a Myth & totally incorrect. It is quite disappointing to keep telling people again and again that spine surgery is safe in 95% cases. A few cases, with spinal cord tumor or have deficits before surgery, then the result are not very good, In my experience 95% of patient walk normally after spine surgery. Why there is improvement in results of spine surgery? This surgery was considered bad 20 years ago. There was a high complication rate. Recently, new investigation methods & operating gadget have improved the results. CT Scan & MRI of spine give us very good anatomical information about bony spine spinal cord, nerves, ligaments & joints. This helps us during the surgery. Operating microscope gives a very good illumination and magnified view during surgery. A small thin nerve also looks like a thick rope. High speed drill to cut bone, CUSA, Suction machine & Cautery machine help us in safe spine surgery. Neuro Monitoring during spine surgery helps to prevent nerve injury. We have seen that spine surgery has become very safe be it for trauma, tumor, degenerative spine disease or congenital anomalies. Speed of spine surgery is also important and reduction in anesthesia time also helps the patient. If a competent spine surgeon does a good surgery in less time, then outcome is better. Lot of research is going on in quality of screws/rods/interbody cages & new minimally invasive procedure are being done. These surgeries have early recovery. Neurosurgeon concentrates more on nerves & spinal cord so does a safe spine surgery. Hence, if you have a spine problem, don't hesitate and go to a good spine surgeon & your surgery will be 99.9% successful if there is not previous injury to spinal cord.

Hi I'm Dr. Rahul Gupta. I will tell you about stroke today. There is a word Brain Attack which is similar to heart attack. In this case the blood supply to a part of brain get cut off or shut down. Brain Attack or stroke is an emergency. It is Ischemic in 85% & Hemorrhagic is 15% cases. In Ischemic stroke, blood supply is cit off there is paralysis of one half of body, Speech problem or facial asymmetry Sometimes, person may be unconsciousness, develop dizziness or blurring of vision or decrease in hearing. This type of stroke need immediate treatment. Patient should be rushed to hospital where facility of CT scan, MRI & Angiography are available. Availability of stroke Neurologist & Neurosurgeon of Neuro-interventionist is essential. The Blocked blood vessel can be opened up by Intravenous medication within Four and a half hours of onset of Stroke. If the patient late, then by intervention procedure in Neuro-cath lab,the vessel can be opened up by aspiration or stent assisted removal (Called as mechanical thrombectomy). By opening the blood vessel, we restore blood flow and prevent morbidity & mortality. Hence, If we suspect stroke clinically at home or at office, We should immediately take patient to an equipped hospital and save his life and prevent major deficits. Post-procedure patient in managed in the stroke ICU and Medication/Rehabilitation is started. Another type of stroke is Hemorrhagic(15%) in Which the blood vessel ruptures and there is bleeding within the brain tissue or outside on the surface. This also needs immediate intervention at an advance Neurosurgical center. CT scan, MRI & Angiography are essential. Depending on type of hemorrhage and its causes, surgical or medical treatment is offered to the patient stroke is a preventable. Major reasons are stressful life, lack of exercise and chronic illness. Control of blood pressure can prevent 90% of stroke. But is spite of that, if a person develops stroke, then immediate intervention at a good hospital can help the patient to attain a healthy life.

We can prevent it by staying away from tuberculosis patient in initial period of illness and take proper treatment. Lung tuberculosis is dangerous and spreads through air. So, let us join hands and help the government to eradicate tuberculosis from India.
