IOPA radiograph was ignored before extraction and hence the lesion real picture was depicted on OPG with more detailed outline on CBCT revealing the true aggressive behaviour on CBCT showing root resorption and massive bone destruction .
Hence the Importance of Diagnosis and proper investigation with correct interpretation.

Beautifully illustrated on CBCT.
Only 5% of cases of dentigerous cysts occur in association with spernumerary teeth i.e., mesiodens in the maxillary anterior region. Dentigerous cysts are one of the most common odontogenic cysts of developmental origin that are usually single in occurrence.It rarely involves incisors and supernumerary teeth.
Complications reported were delayed or prevented eruption of succedaneous teeth, displacement or rotation, crowding of the affected region, abnormal diastema, dilacerations, cystic formation, and sometime eruption into the nasal cavity.

Osteomas are benign osteogenic lesions with very slow growth, which may arise from proliferation of either cancellous or compact bone. In the jaw, these are uncommon lesions.
Osteoma consists of compact or cancellous bone that may be of peripheral, central, or extraskeletal type. Peripheral osteoma arises from the periosteum and is commonly a unilateral, pedunculated mushroom-like mass.
Although multiple osteomas of the jaws are a hallmark of Gardner’s syndrome (familial adenomatous polyposis), nonsyndromic cases are typically solitary.

CBCT view revealed a well-defined radio-dense area attached to the lingual aspect of mandible .
Very well documented on Axial Reformation and Cross-sectional views. The 3D reconstruction of the CBCT image revealed a pedunculated homogeneous bony mass attached to the Lingual cortex at the right body of the mandible irt #44, #45.

Overall, CBCT scans are significantly more accurate than dental radiographs in detecting root fractures .The sensitivity of CBCT scans, have been calculated in various studies for detecting root fractures to be significantly higher than that of dental radiographs . The specificity was considered high and comparable for both dental radiographs and CBCT scans.
Detection of vertical, oblique, horizontal tooth fracture precisely observed on CBCT.
