Back pain is very common in pregnancy but true leg pain (sciatica) due to slipped disc (disc herniation) is extremely rare and most of the slipped disc will heal so pregnancy take its course without needing spinal surgery

This 31 year old 16 weeks pregnant lady carrying her first precious baby presented with few months of right leg pain due to slipped disc. She was not able to sit or sleep in any comfortable position.

Decision taken to wait till 20th week anomaly scan which showed normal foetal development and then she was offered spine surgery to remove the disc

Second trimester (4th-6th month) of pregnancy is safe for any essential and emergency surgeries

When it comes to spine surgery in pregnancy two most challenges are how we position without pressure on baby and how we confirm we are operating on right disc level. Normally x-rays are taken during surgery to confirm we are operating on right disc level as wrong level surfer is most common complication in spine surgery.

Our team felt the need for surgical strategy to be tailored to mother and foetal well being

Positioned prone on high bolsters so baby bump is completely free

Level localisation without use of Intra op X-ray to avoid teratogenic effects on growing baby was decided.

Ultrasound was used to localised the L4/5 level and gadolinium contrast soaked gauze was placed on the localised surface and this was confirmed on screening MRI subsequently incision was taken and we ended in the right disc space fishing out multiple huge disc

The day after surgery she said she is able to sit stand and sleep in comfortable position and her smile showed she started to enjoy her pregnancy pain free

DO NOT IGNORE BACK PAIN IN CHILDREN

Back pain in younger children is a red flag symptom indicating underlying serious spinal problem . But it’s often get ignored by parents or non speciality physicians assuming it’s growing pain or postural or backpack related.

Back pain in children is the only earlier symptom and if ignored can quickly destroy the spine bone leading to deformity and paralysis of limbs.

Early advanced MRI of spine will help pick the infection at early stage and tackle it with anti tuberculosis medication by which invasive surgery can be avoided

Here is 6 year aged child whose back pain been ignored for 6 months presents severe bone destruction needing major surgery to help recover from this medieval disease

June is about to end and 10 year old Parameshwari is here to speak about very very special type of scoliosis she is suffering from.

Neuromuscular scoliosis is a type of scoliosis that can occur in children who have medical conditions that weaken their ability to control the muscles that support the spine.

Children with muscular dystrophy, cerebral palsy and spina bifilar develop neuromuscular scoliosis.

Most of these children are non ambulators and spend most time sitting in wheelchair. When scoliosis develops it will affect their sitting balance.

Brace and accommodative wheelchair will help when scoliosis is mild and surgery will be needed to improve sitting balance when the curve is severe.

Neuromuscular scoliosis is the most challenging amongst all types of scoliosis and managed by multidisciplinary team.

Early detection and appropriated treatment will improve the quality of life of these special needs children

Public health begins with public awareness. For disease prevention, improved population health, and all of the other goals of public health, the single most important thing is information. Governments cannot make people healthier by legislation, and public health professionals cannot save people who do not know they are in danger. Awareness is critical to public health.

Medical public awareness campaigns are designed to distribute important healthcare messages to large populations.

June is official Scoliosis awareness month and strives to spread awareness about the condition itself, it’s prevalence and the merits of early detection and benefits of proactive treatment

Its time for two of our youngest scoliosis warriors to speak up about special type of scoliosis which presents from birth called “Congenital Scoliosis” “Early Onsent Scoliosis”.

Congenital scoliosis is a sideways curvature of the spine that is caused by a defect in the formation or separation of spine bone (vertebra) that was present at birth.

It occurs 1 in 10,000 newborns and is much less common than the type of scoliosis that begins in adolescent age.

Children with congenital scoliosis sometimes have other health issues, such as kidney or bladder or heart problems because all organs tend to form during 4-8 weeks of embryonic development process called organogenesis

Even though congenital scoliosis is present at birth, it is sometimes impossible to see any spine problems until a child starts walking or later on during growth spurts.

Though less common Congenital Scoliosis is most challenging to manage. Early detection and intervention will help proper spinal and chest cavity growth which is essential for lung development in growing child.

If Congenital scoliosis is left untreated will lead to poor lung function and reduced quality of life and reduced life span.

If detected early plaster casting, braces and surgeries will help normal development of spine and lungs.

Both warriors were born with scoliosis and had surgeries at age of 6 years and it’s very important to follow them up until they grow into adult to monitor the behaviour of the curve and consider additional interventions if needed.

Best wishes for my both scoliosis warriors
