👉DADC twins
👉IVF pregnancy
👉Rescue Encirclage applied at a Private Nursing home on 24/5/23 for cervical incompetence with bag of membranes bulging through internal os (picture attached)
👉Cervical stitch removed next day on 25/5/23 at the same Private Nursing Home at 23+2 weeks and referred to Cloudnine hospital Panchkula

At Cloudnine Hospital Panchkula
👉Conservative management given
👉Exactly after 8 weeks of conservative management patient underwent Emergency LSCS in view of PPROM on 17/7/23 At 31 weeks
Twin 1 - Girl (1.48 kg)
Twin 2 - Boy (1.4 kg)

Both babies and mother doing fine

👉Previous 2 LSCS (Both Girls)
👉History of secondary infertility since last two years
👉Previous history of unsafe mid trimester abortion done at some peripheral hospital in Haryana after s*x determination
👉TVS - suggestive of Retained Bone fragments / Asherman’s

On Hysteroscopy
👉Endocervical canal negotiated with difficulty
👉Entire uterine cavity obliterated with bad Ashermans
👉Intrauterine adhesions released with scissors
👉Multiple Bone fragments deeply embedded into the walls of the endometrial cavity
👉Same removed with grasper
👉Post procedure cavity was restored to a large extent
👉Patient counseled about prognosis
