Challenges with TURP Positioning in Patients with Hip Issues

Positioning a patient for TURP (Transurethral Resection of the Prostate) can be challenging when there are underlying hip conditions. Stiffness, arthritis, or a history of hip surgery may limit the patient’s ability to comfortably assume the lithotomy position—especially when the buttocks must be flush with the edge of the table for optimal access to the bladder and prostate.

A multidisciplinary approach, involving urology, anesthesiology, and help of ground staff when necessary, can help ensure both safe positioning and procedural success.

I had the privilege of performing a challenging case—a successful laparoscopic removal of a large right adrenal myelolipoma (15cm!) in a 50-year-old woman from West Bengal. The tumor was sizable and tightly adhered to the right kidney, which was pushed downward and medially, with the right renal vein draping over it.
