We feel honored to share an incredible success story from North End Hospital — a case that speaks volumes about the expertise, courage, and teamwork of our medical professionals.
An 85-year-old female patient was admitted to our hospital for a planned cholecystectomy. What made this case extremely high-risk was her complex medical condition. She was a known case of Type 2 Diabetes Mellitus, suffering from severe Pulmonary Artery Hypertension (PAH) with a Right Ventricular Systolic Pressure (RVSP) of 70 mmHg, and was showing clear signs of right heart failure.
At presentation, the patient was dyspneic and hypoxic, with an oxygen saturation (SpO2) of just 77% on room air. Despite these serious challenges, her ejection fraction (EF) was preserved at 58%, indicating reasonably maintained left heart function.
Before proceeding to surgery, a joint decision was taken by our expert anesthetist Dr. Shabir Ahmad and cardiologist Dr. Mustafa Bashir to optimize the patient’s condition. Patient was put on Diuretics and Tab seldenafil.

Managing such a fragile patient required meticulous planning and seamless coordination between departments.
Dr. Shabir Ahmad, our highly skilled anesthetist, made the critical decision to proceed with thoracic spinal anesthesia, a bold and precise choice that helped avoid the risks associated with general anesthesia in such a compromised patient. His intraoperative management was nothing short of exceptional, especially considering the ongoing anesthesia-related challenges both during and after the procedure.
On the surgical side, Dr. Gulzar Ahmad performed the cholecystectomy with absolute focus and finesse, ensuring minimal hemodynamic disturbance throughout the surgery.
Thanks to the collective effort, advanced planning, and professional brilliance, the surgery was completed safely, and the patient was successfully managed postoperatively.
