25 yr Indian female (5ft 2) with pain and heaviness in right leg since 3 weeks after lifting heavy weight. Target - AWMZ, AWLZP1. Challenges - huge posteriorly migrated disc with difficulty in reaching via transforaminal route. Did right L5S1 transforaminal endoscopy UNDER LOCAL ANAESTHESIA. Entry point 14 cm. Reached midline at the level of Posterior vertebral (traditionally at midline, needle should be in posterior 1/4th inside disc). Facet drilling was not needed. Medialisation of annulotomy. Big disc fragment removed. Surgery time 30min. No stitches. Patient walked 2 hrs after surgery. No leg pain
