This middle aged lady presented with chronic back pain and early difficulty in walking. There was past history of fall which led to wedge compression fracture. Now her MRI showed kyphotic deformity with myelomalacia which was due to draping and rubbing of the cord over the retropulsed fragment. Her kyphosis correction was done and had remarkable recovery.Though the old fractures have healed but can give delayed complications.. This 45 yrs old patient developed paralysis in all 4 limbs after an accident. Since the touch sensation was preserved we had some hope for his recovery. MRI was showing an altered signal in spinal cord and no cord compression which led to the diagnosis of a whip lash injury .On a closer look we found that his spine was not properly aligned and suspected an instability which was confirmed with dynamic Xrays. His spinal cord was swollen significantly and early surgery would have been more damaging .We treated him conservatively with advanced physiotherapy for next 3 months till he recovered significantly. His surgery was then planned and executed by anterior Decompression of spine and fusion by bone graft followed by fixation. Patience is the key in neurosurgery and performing surgery at a right time gives wonderful results. . This patient presented with loss of balance while walking and hence needed support of someone when he came to me. On investigation he was found to have Tuberculosis of Cervical spine which showed significant compression of spinal cord. Since the cord compression was from front(anterior) of the spinal cord therefore I advised him surgery will be done from the front of the neck to which patient got scared that it would be very risky surgery. He was then explained in detail about surgery and then he got convinced and surgery was performed. He went home walking with a happy face. Anterior surgery in the neck is a very safe surgery at hands of experienced surgeon . Malignant tumor need aggressive removal of tumor at the first surgery itself and this can give a good long term prognosis .This patient had grade 3 Malignant tumor and i had operated upon him around 41/2 yrs back now developed recurrence and again was recently operated again for the same .This time Neuronavigation helped for better localisation and total clearance of the tumor .He had SMA Syndrome post op but improved completly. .

Many a times this Hypertension goes unnoticed as people think that they are young and cannot have Hypertension and one day they land up in acute crisis of brain hemorrhage and paralysis. Chronic subdural hematoma in a very elderly patient is a very tricky situation especially when it has an associated subdural membrane .There is a chance of recurrent collection or postoperative bleeds as these patients most of the times are on ecospirin which leads to platelet dysfunction. Moreover we have only one chance to treat them as patient being very elderly may not tolerate repeated surgeries. However surgery at hands of experienced surgeon can give wonderful results. . Tethered cord syndrome is a congenital abnormality in the new born babies in which spinal cord is adherent to the structure outside the spinal canal therefore as the child grows the spinal cord gets pulled and leads to various irreversible symptoms .Therefore early surgery is advised.Such complex surgeries can be done with intraoperative monitoring of spinal cord signals while performing surgery .
