Diabetic Macular Edema (DME) is a complication of diabetic retinopathy where fluid leaks from damaged blood vessels in the retina and collects in the macula (the central part of the retina responsible for sharp vision). This swelling causes blurred or distorted central vision and can lead to vision loss if untreated.

Ozurdex is a tiny biodegradable implant that slowly releases dexamethasone (a corticosteroid) inside the eye. It helps reduce inflammation, decrease fluid leakage, and shrink swelling in the macula. It is injected directly into the eye by means of a painless, short procedure

Benefits: Improves vision, reduces macular thickness, longer duration of action (up to 3–6 months

👉 It is usually considered in patients not responding well to anti-VEGF injections or when steroid therapy is preferred.

Cataract surgery in patients with kyphosis (forward bent spine) or scoliosis (curved spine) can be more challenging mainly due to positioning difficulties. Normally, patients lie flat on their back, but spine deformities make this uncomfortable or impossible. As a result, surgeons may need to tilt the operating chair, use extra cushions, or adjust the microscope to get a proper view of the eye.

These posture issues can also affect breathing, so anesthesia must be carefully planned, often with local anesthesia rather than general. Because of the awkward head position, the surgeon may face a less stable view and restricted access to the eye, making the procedure technically more demanding and sometimes longer.

In short, the biggest challenge is achieving a safe, comfortable position for both patient and surgeon. With careful adjustments and planning, cataract surgery in such patients is usually safe and effective.
