अगर आपकी fibroid surgery (myomectomy) हो चुकी है, तो यह सवाल आना बिल्कुल normal है, delivery normal होगी या cesarean?

जब baby का weight expected से कम होने लगे, तो timely monitoring से उसे safe रखा जा सकता है।

अगर scan में baby का weight expected से कम दिखे,
तो इसका मतलब हो सकता है कि baby की तरफ जाने वाला blood flow कम हो रहा है।

इस condition को कहते हैं Fetal Growth Restriction (IUGR)।

IUGR में baby survival mode पर चला जाता है
brain और heart को ज्यादा blood मिलता है
पेट (abdomen) की तरफ blood flow कम हो जाता है

इसलिए ज़रूरी होता है:
• Regular sonography
• Doppler study से blood flow check करना
• NST monitoring से heart rate देखना
• Amniotic fluid (baby ka pani) पर नजर रखना

अगर monitoring में लगे कि baby uterus के अंदर safe नहीं है, तो timely delivery baby के लिए ज्यादा सुरक्षित option होता है।

Pregnancy test positive होने के बाद भी जब baby develop नहीं होता, तो उसे Blighted O**m कहते हैं।

15 दिन पहले urine pregnancy test positive था।
Beta hCG levels हर 48 hours में properly doubling हो रहे थे।
Gestational sac uterus के अंदर confirm थी।

लेकिन viability scan में यह पता चला कि fetal pole absent है, इस condition को Blighted O**m (Anembryonic Pregnancy) कहा जाता है।

Blighted O**m के common reasons:
• Egg and s***m chromosomal defects (most common)
• Hormonal imbalance जैसे low progesterone
• Thyroid disorders
• Lifestyle factors: smoking, alcohol
• Maternal age factor

Low AMH का मतलब उम्मीद खत्म होना नहीं, सही diagnosis से रास्ता निकलता है।

इनकी उम्र 32 साल थी और AMH Low था।
हर cycle में सिर्फ one follicle develop होता था और oocyte quality भी अच्छी नहीं थी।

हमने उन्हें ovarian rejuvenation के options बताए

Heartbeats होने के बावजूद, कभी-कभी pregnancy uterus में नहीं, cervix में होती है।

इस case में pregnancy test positive था और heart activity present थी, लेकिन ultrasound में पता चला कि यह cervical pregnancy थी, यानी pregnancy गर्भाशय के अंदर नहीं, बल्कि cervix में थी।

ऐसी pregnancy में heavy bleeding का risk ज्यादा होता है, क्योंकि cervix में muscles नहीं होतीं जो bleeding को खुद रोक सकें।

समय पर ultrasound-guided evacuation (D&E) किया गया और bleeding रोकने के लिए balloon compression लगाया गया।

24 घंटे में bleeding control हुई और patient safely recover हो गई।
सीख यह है:
• Early ultrasound बहुत ज़रूरी है
• Bleeding को ignore न करें

AMH कम होने का मतलब यह नहीं कि IVF में सफलता नहीं मिलेगी।

पहले बच्चे में cleft lip palate था, तो क्या अगली pregnancy में भी वही वापस होगा?

7 weeks complete हो चुके हैं और baby heartbeat आ चुकी है|

आपका डर बिल्कुल समझ में आता है, क्योंकि पहले baby को cleft lip और cleft palate था और multiple surgeries से आप गुजर चुकी हैं।

सब कुछ normal होते हुए भी pregnancy न रुक, तो IVF सही अगला कदम हो सकता है 🌸

PCOS होने के बावजूद कई बार IUI cycles में अच्छे follicles, normal fallopian tubes और normal semen parameters होने के बाद भी pregnancy नहीं रुकती।
ऐसा इसलिए क्योंकि कई microscopic reasons होते हैं जो reports में दिखाई नहीं देते।

इस case में हमने IVF with minimal stimulation का फैसला किया, और result शानदार रहा

30 oocytes मिले, जिससे अच्छे quality embryos बनने की strong possibility है और future में repeat o**m pickup की ज़रूरत भी कम हो जाती है।
