HPV positive report का मतलब हमेशा cancer नहीं होता, सही जांच और follow-up ज़रूरी है।

32 साल की patient बिल्कुल ठीक थीं। कोई दर्द नहीं, कोई bleeding नहीं। Routine checkup में Pap smear और HPV DNA test कराया… और रिपोर्ट HPV positive आई।

डर लगना स्वाभाविक है। लेकिन अगला सही कदम है Colposcopy एक simple OPD procedure जिसमें microscope से cervix को detail में देखा जाता है। यह surgery नहीं है, anesthesia की जरूरत नहीं होती।

इस case में lesion छोटा और mild था। Cancer नहीं था। केवल regular follow-up की सलाह दी गई।

हर HPV report cancer नहीं होती। लेकिन HPV positive होने पर evaluation ज़रूरी है। Early screening ही protection है।

Low s***m count और motility में सही technique चुनना ही सफलता की कुंजी है।

इस couple ने पहले 10 IUI cycles कराए, लेकिन reports में male factor infertility थी। जब s***m count और motility कम हो, तो IUI के chances बहुत सीमित होते हैं।

Clear counselling के बाद ICSI (Intracytoplasmic S***m Injection) recommend किया गया। Egg retrieval में 5 mature eggs मिले। हर egg में single healthy s***m inject किया गया। Fertilization अच्छा रहा और 4 healthy blastocysts बने।

Family में single gene disorder हो तो IVF with PGT-M से healthy embryo चुनना संभव है।

अगर परिवार में genetic disease है, तो natural conception संभव है। लेकिन pregnancy के बाद baby affected है या नहीं, यह जानने के लिए chorionic villus biopsy (CVS) या amniocentesis जैसी जांच करनी पड़ती है।

अगर baby affected हो, तो आगे कठिन निर्णय लेने पड़ सकते हैं।

एक और विकल्प है, IVF with PGT-M (Preimplantation Genetic Testing for Monogenic disorders)। IVF में embryos बनाकर day-5 stage पर genetic testing की जाती है। Healthy embryos को select करके transfer किया जाता है।

14 cm का बड़ा fibroid होने के बावजूद सुरक्षित pregnancy और healthy delivery संभव है, सही planning के साथ।

Urine pregnancy test positive था, लेकिन पहली sonography में 14 cm का बड़ा fibroid सामने आया। उम्र 36 साल और पहली pregnancy, इसलिए termination का सवाल ही नहीं था।

28 weeks तक pregnancy stable रही। 32 weeks पर detailed evaluation, hemoglobin कम था तो injections दिए गए। Baby growth अच्छी रही।

Embryo अच्छा होने के बावजूद pregnancy न ठहरे, इसे Implantation Failure कहते हैं।

कई बार embryo healthy होता है, लेकिन uterus में सही तरह से implant नहीं कर पाता।

इसके कई कारण हो सकते हैं:
• Endometrium lining ठीक से तैयार न होना
• Implantation window में progesterone timing का mismatch
• Uterine polyp, fibroid या adhesions
• Hormonal imbalance

Repeated D&C के बाद periods कम हो जाएं और pregnancy न हो, तो अंदर adhesions हो सकते हैं।

शादी के बाद unwanted pregnancy के लिए D&E हुआ। फिर दो बार pregnancy loss हुआ। तीन बार D&C के बाद periods कम हो गए और pregnancy रुक गई।

जांच में पता चला Asherman Syndrome, यानी uterus के अंदर चिपकाव (adhesions) बन गए थे।

Hysteroscopy के माध्यम से दूरबीन डालकर उन adhesions को हटाया गया। अंदर की lining दोबारा healthy बने इसके लिए दवाइयाँ दी गईं। 2–3 महीने बाद endometrium बेहतर हुआ… और patient ने natural pregnancy conceive की।

अगर D&C या uterine surgery के बाद periods कम हो रहे हैं या pregnancy में दिक्कत है, तो evaluation ज़रूरी है। Early diagnosis से treatment सरल और सफल हो सकता है।

जब सारे reports normal हों, फिर भी pregnancy न हो, उसे Unexplained Infertility कहते हैं।

यह कपल 4 साल से conceive करने की कोशिश कर रहा था। Periods regular, ovulation normal, semen analysis normal, tubes open… फिर भी pregnancy नहीं हो रही थी। ऐसी स्थिति को Unexplained Infertility कहा जाता है।

Age और duration को ध्यान में रखते हुए IVF plan किया गया। Stimulation smooth रहा, 4 oocytes मिले और उनमें से 1 healthy day-5 blastocyst तैयार हुआ। Embryo transfer सरल रहा, और 15 दिन बाद beta hCG positive आया। Ultrasound में pregnancy sac भी confirm हुआ।

पहला baby C-section से हुआ है… इसका मतलब यह नहीं कि हर बार surgery ही होगी।

अगर आपका सिर्फ एक previous cesarean हुआ है, uterus के lower segment में incision था, pregnancy low risk है और baby का weight ज्यादा नहीं है, तो VBAC consider किया जा सकता है।

हर heavy bleeding का solution surgery नहीं होता, सही diagnosis सबसे जरूरी है।

Irregular periods, heavy bleeding, hormonal imbalance, progesterone therapy, hormonal IUD treatment, कई बार reports normal होती हैं, लेकिन periods फिर भी गड़बड़ रहते हैं।

42 साल की patient को कभी 20 दिन तक bleeding, तो कभी 2–2 महीने gap। Ultrasound normal, biopsy normal, कोई fibroid, polyp या adenomyosis नहीं। Diagnosis था Hormonal Imbalance।

उन्हें लगा surgery ही एक रास्ता है, क्योंकि दो जगह ऐसा बताया गया था।
लेकिन सही evaluation के बाद progesterone treatment शुरू किया गया और बाद में hormone releasing intrauterine device (IUD)लगाया गया।

6 महीने बाद bleeding control में, और life normal हुई ।
