Why Osteoarthritis?
There are three different types of cartilage found in the body: articular or hyaline cartilage (covers joint surfaces), fibrocartilage (knee meniscus, vertebral disk), and elastic cartilage (outer ear). These different cartilages are distinguished by their structure, elasticity, and strength.

ARTICULAR CARTILAGE is a complex, living tissue that lines the bony surface of joints. Its function is to provide a low friction surface enabling the joint to withstand weight bearing through the range of motion needed to perform activities of daily living as well as athletic endeavours. In other words, articular cartilage is a very thin shock absorber. It is organized into five distinct layers, with each layer having structural and biochemical differences.

Articular cartilage has a very limited capacity for self-repair. Small damage does not repair itself and can often get worse over time. As cartilage is aneural and avascular (lack of nerve and blood supply, respectively), shallow damage often does not trigger pain.

Osteoarthritis

Articular cartilage can be injured by an injury (trauma) or wear & tear degeneration.

Because articular cartilage has no direct blood supply, it has little or no capacity to repair itself. If the injury penetrates the bone beneath the cartilage, the underlying bone provides some blood to the area, improving the chance of healing.

When there is degeneration of articular cartilage due to wear and tear, there is damage and loss of cartilage structure and function.

The cartilage initially softens, then later on progresses to fragmentation. Due to the continuous loss of the articular cartilage, the underlying bone has no protection from the normal wear and tear of daily living and begins to break down. This leads to osteoarthritis.

Bilateral Knee Replacement:

The risk of blood clot after a surgery tends to be slightly higher for patients of bilateral knee replacement surgery than it is for those having surgery on just one knee.

Speak to your surgeon and find out all about the signs and symptoms so you can report them to her/ him, in case they occur. the risk for developing a blood clot stays up for at least two months after the operation. There are many kinds of blood clots, and some are more serious than others. More commonly, blood clots develop after knee or hip replacement surgery in veins in the leg. This usually affects the operated leg, but clots can also develop in the other leg. In some cases such clots travel to the lungs.

Blood-thinning medication As there’s no method to predict who is at highest risk for developing blood clots after surgery, all patients having knee or hip replacement are given “anticoagulants” so that it is less likely for the blood to clump together (clot). You may be told to keep taking these pills ( in correct dose) for some weeks after surgery or longer.

IMPORTANT: REMEMBER TO UNDERSTAND AND WRITE DOWN ALL POST - OPERATIVE CARE INSTRUCTIONS EXPLAINED BY YOUR SURGEON. DO NOT ASSUME THINGS OR TRY TO TREAT THE PATIENT.

How are ACL tears treated surgically?
Current techniques involve reconstructing the ACL by building a new ligament out of tissue harvested from one of the other tendons around the knee or from an organ donor. This tissue is passed through holes Are drilled into the thigh bone and shin bone and the tissue is passed through them, which are then tied in place to create a new ACL.
Over the time this graft matures and becomes a new, living ligament in your knee.
Recovery from ACL injury
Recovery may take 6 weeks to 6 months depending on the activity level of the patient.
The success rate for ACL surgery is very good. For optimum results physiotherapy and rehabilitation in thecorrect manner is very significant.
Many studies have shown that more than 90 percent of patients are able to return to sports without symptoms of knee instability. Although some patients do complain of stiffness and pain after surgery, these problems have been minimized by current surgical
techniques and aggressive rehabilitation.
