When 2D lies… CBCT reveals the truth.
A young female patient reported pain in the upper left posterior region.
Clinically, the maxillary third molar appeared palatally positioned and fused with the second molar.
🩻 2D radiograph suggested fusion of crowns and roots
🔍 But CBCT told a different story — no fusion at all
Instead:
Upper 8 → Deep occlusal caries
Diagnosis → Irreversible pulpitis + apical periodontitis
✅ Treatment: Selective extraction of 28 under LA
❌ 27 preserved — avoiding unnecessary intervention
Takeaway:
Not everything that looks fused is truly fused.
👉 CBCT can be the difference between overtreatment and precision dentistry
