A 53-year-old male presented with complaints of lower abdominal pain. On evaluation, imaging revealed an ectopic left kidney located in the pelvis with a staghorn calculus measuring 3 × 3 cm, associated with moderate hydronephrosis. After appropriate preoperative workup, the patient was planned for and underwent laparoscopic pyelolithotomy. The procedure was successfully performed, and a double-J (DJ) stent along with an abdominal drain was placed intraoperatively. The per-urethral catheter was removed on postoperative day (POD) 2, followed by removal of the abdominal drain on POD 3. The postoperative course was uneventful, and the patient was discharged on POD 4 in stable condition, asymptomatic, and without any complaints.

Impacted Ureteric Stone – A Painful Block That Needs Timely Action

The ureter is a thin tube that carries urine from the kidney to the bladder. When a stone travels from the kidney and gets stuck in the ureter, it is called an impacted ureteric stone. This condition is quite painful and can lead to serious complications if not treated on time.

⚠️ Symptoms to Watch For:

Severe pain in the flank or lower abdomen

Nausea or vomiting

Difficulty or burning while urinating

Blood in urine (reddish or brown color)

Fever or chills (if infection develops)

🧠 Why It Happens:

Impaction usually occurs when a stone is too large to pass naturally or gets lodged in a narrow part of the ureter. Dehydration, dietary habits, or metabolic disorders can increase stone formation risk.

🩺 Treatment Options:

Modern urology offers several safe and effective treatments such as:

URS (Ureteroscopy): A thin scope is passed through the urine passage to break and remove the stone.

As a urologist, I often encounter patients who have stones in both kidneys (bilateral nephrolithiasis) or both ureters (bilateral ureterolithiasis). One of the most common questions they ask is — “Doctor, which side should be operated first?”

In cases of bilateral ureteric stones, the side with more severe obstruction, infection, or pain must be addressed first.
