55/M ,come to opd with 7-10 different files , 4-5 CT scan and complains of cough since 2 year , progressive and now developed dyspnoea since 2-3 months.. Radiological imaging suggestive of Non resolving right lower lobe pneumonia since 2 year.
He had undergone extensive workup for vasculitis , malignancy, TB including bronchoscopic biopsy ,CT guided Biopsy but nothing were came positive and still worsening by symptoms and radiologically (now having mild Loculated effusion with consolidation progresses).
He was admitted multiple times and also taken AKT for the same.

After evaluation & stabilization of dyspnoea we have done TRUCUT Transthoracic LUNG biopsy from consolidation part which was completed without any pneumothorax .

Biopsy - Bronchial Mucosa associated lymphoma - BALTOMA (Rare type of Extra nodal marginal cell lymphoma)

Differential diagnosis of Non resolving Pneumonia -

Tuberculosis
Fungal infection
Malignancy
Wegner's granulomatosis(vasculitis etiology)
Bronchoalveolar cell carcinoma
Lymphoma
Post obstructive pneumonia
