Nonarteritic anterior ischemic optic neuropathy (NAION) is the second most common optic neuropathy after glaucoma caused by infarction of the short posterior ciliary arteries that supply the anterior portion of the optic nerve head. This leads to axonal edema (seen in left eye image) and a compartment syndrome in an already crowded optic disc causing vision loss.
Systemic diseases that may cause decreased perfusion to the optic nerve head secondary to microvascular compromise might increase the patient’s risk of NAION. These include hypertension, diabetes, and hypercholesterolemia.
