Care Continuum was conceived & founded in 2014 as a social enterprise, to make a significant difference to the on-demand model of Home Healthcare practised by a majority of the providers till date. We believe that health services at home require proper clinical planning and implementation to minimise the risks and prevent frequent hospitalisations.
