A low body weight Child C CLD with PHTN with past history of bleed , encephalopathy and Jaundice with Multiple EVL for large varices and PHG was referred with severe pain abdomen and early sepsis with tense ascites .
Further tests revealed acute Appendicitis and cholelthiasis . Clinically Only Macburneys was tender with other signs positive and tender right hupochondrium . We admitted few days ahead of unavoidable surgery after having walked into many plans and alternatives . Daily paracentesis was done under Albumin coverage to attain temporary dry tap . High antibiotics and other measures didnt reduce pain and tenderness. Fluid studies ruled out SBP . Repeat UGI EVL etc were done .
All pros and cons discussed with relatives and as pain was increasing we had to decide for laparoscopy and proceed .
There was severe collaterals and caput and grossly adherent Right colon with multiple bands causing SAIO and after too painstaking mm by mm dissection , could reach an inflammed retrocaecal high up inflammed appendix . But both surgery could be competed laparpscopic and with almost no blood loss . No hemodynamic changes on table . Gallbladder surgery also went uneventful.

1 . Switch off all lights in OT and press 100 % light source into parietes , will show mesh of Collaterals with small avascular windows to enter abdomen.

2 . Choose ports as balance between collaterals and ergonomic of Surgeons movement needed

3 . Its not necessary inspite of bad vessels , there will be bleed if done by mm s and with liberal use of clips , hemolocks , ties , energy sources .

4 . Preventing bleed is better than controlling ( not absolute but worth trying always )

5 . Smallest oozes should be dealt with immediately ( pressure on any soft areas eg liver or fat or omentum is better than diathermy as actually bleed may increase. Hold with ligasure where u can . Else simple coag with open jaw of Harmonic preferably at 1 or 2 minimum )

6 . All instrument entry to exit movement should be under vision to avoid even slightest trauma to any tissue by mistake - even parietes can bleed out ( worse is , bleed even controlled can lead to Liver failure due to hypotension )

6 . Even ports should be closed under vision of camera for surety . Last 5 mm or 10 mm under vision of eyes .

Per op - IAP , CVP , MAP , PEEP , CO2 level , Urine , Fluid , and recruitment plays an equally important role

Keep Challenging Yourself within scientific calculated risk limits to learn new , do better and making new benchmarks for self , especially if there is even ONE WINDOW to TRY to save someones Life . 🙏🔱🙏

As Human I can Only TRY. Shiva can do the rest . ❤️

Operating on Cirrhotic liver patient with Portal hypertension is very challenging

A radical Cholecystectomy for Gallbladder cancer also very challenging

Doing both is really stressful

As any bleed may not stop or clot easily
Liver can fail at any point
And many other critical issues

Patient had bad heart , sugar , BP and other conditions

But fortunately, keeping fingers crossed , went smooth with GODS GRACE , FANTASTIC ANAESTHETIST AND TEAM and LORD SHIVA 🙏

Hardly any blood loss so no transfusions were needed .

Too early to be happy but satisfying to complete in short time, minimal blood loss or any hemodynamic changes yet do as complete surgery as possible .

Was Invited to give a lecture on Pancreatic Cancers at the new formed academic institution by Meril at Kolkata near Sector 5 .

I talked on my series of Whipples Pancreatoduodenectomy and its results .
There were other esteemed speakers from different surgical specialities .

But 3 things really touched me

1. A Gujarat Vapi based life sciences company has formed its 1st Training and Academic Institute here which will benefit lot of trainees and others .

2. A very lovely set up and hospitality especially a young Man named Rohan who accompanied me to guide me as I am self driven but not very savvy with all roads on that side ( nor do I always feel comfortable with maps especially going to a part relatively new to me and parking issues which was also solved by him )

3 . The very very thoughtful MEMENTO as felicitated to the dignitaries and speakers - The Ashok Stambh curved out of wood . ( REALLY REALLY FELT HONORED AND LOVED IT 😍)
