Do you see patients with a diagnosis of carpal tunnel syndrome?
If a nerve conduction study to "confirm" this diagnosis is positive do you send the patient for a surgical opinion?
Have you tried to explore repeated movements of the cervical spine or wrist to see if you can change these symptoms?
This study found:
38 patients with carpal tunnel syndrome performed repeated movements following which electrophysiological measurements were made. 32% of hands worsened with flexion and extension movements; 22% of hands improved with extension movements and some improved with flexion.

Derangement is one of the MDT classifications in both the spine and extremities. It is characterised by rapid change and directional preference, where repeated movements in one direction rapidly relieve pain and restore ROM/power, and movement in the opposite can rapidly worsen it.
When this is found in a knee or elbow joint how to we explain this to the patient? What structure do we think is implicated?
Robin McKenzie's reply to one of his patient's who had an elbow derangement when asked about what was "getting in the way" was "I don't know, but something is!".
This study reveals structures like meniscoids and fatpads inside the elbow.
