01/09/2025

Why TAPP(TRANSABDOMINAL PREPERITONIAL REPAIR) CONSIDER SUPERIOR TO OTHER METHOD OF UMBILICAL HERNIA SURGERY FOR SMALL TO MEDIUM SIZE DEFECT(IPOM ,TARM)

1️⃣ Understanding the techniques
TAPP (TransAbdominal PrePeritoneal repair)
Access through the abdominal cavity β†’ create a peritoneal flap β†’ place mesh in the preperitoneal space β†’ close the peritoneum over the mesh.

IPOM (IntraPeritoneal Onlay Mesh)
Place mesh inside the peritoneal cavity directly over the defect β†’ mesh in contact with abdominal contents β†’ often requires special coated mesh to reduce adhesions.

TARM (TransAbdominal RetroMuscular repair)
Access extraperitoneal retromuscular space through laparoscopy β†’ place mesh between posterior re**us sheath and muscle, mimicking the open Rives–Stoppa technique.
--

2️⃣ Why TAPP is sometimes considered superior for umbilical hernia
βœ… Mesh Position & Protection
TAPP: Mesh is placed outside the peritoneal cavity (preperitoneally) β†’ bowel is not in direct contact with mesh β†’ reduces risk of adhesions, erosion, fistula.
IPOM: Mesh is intra-abdominal, so bowel contact is unavoidable (even with composite mesh, adhesion risk exists).
TARM: Similar advantage as TAPP in avoiding bowel contact, but it's technically more complex for small defects like primary umbilical hernia.

βœ… Anatomical Reconstruction
TAPP allows closure of the defect and reinforcement in the correct anatomical layer.
Prevents β€œbridging” and gives better functional abdominal wall outcomes.

βœ… Lower Risk of Chronic Pain
Preperitoneal dissection in TAPP avoids placing fixation devices in nerve-rich areas and can allow for less traumatic fixation.

βœ… Material Cost
TAPP can use standard uncoated polypropylene mesh (cheaper πŸ’°).
IPOM requires expensive composite/barrier mesh to reduce adhesion.

βœ… Recurrence Rates
Mesh in the sublay/preperitoneal plane distributes forces better β†’ lower recurrence potential compared to onlay positions.
--

3️⃣ Why not always TAPP?
Technically more demanding: Requires careful preperitoneal dissection and peritoneal closure
Time: Longer operative time compared to standard IPOM in less experienced hands.

Training needed: Not all centers may have the required laparoscopic expertise for ventral preperitoneal repairs.

πŸ‘‰ Bottom line:
TAPP for umbilical hernia can be superior to IPOM due to lower adhesion risk, lower mesh cost, and more anatomical placement, and it is technically simpler than TARM for these defects.
However, it depends on the surgeon’s skill β€” in less experienced hands, IPOM remains faster and easier. Precision matters here! βš–οΈ

Quick facts and myth-busting

Q-Are gallstones more common in women? Yes. Gallstones are more common in females (especially with pregnancy, hormone therapy or oral contraceptives), and with increasing age and obesity.

Q-Are small stones more dangerous than large ones? It depends. Small stones are more likely to slip into the bile duct and cause serious problems like blockage, cholangitis or pancreatitis. Large stones may cause chronic irritation or symptoms too.
Bottom line: both small and large stones can be problematic.

Q-Do long-standing gallstones cause major problems? They can. Untreated gallstones may lead to repeated gallbladder inflammation (cholecystitis), bile duct infection (cholangitis), pancreatitis, and rarely increase cancer risk. Many people have asymptomatic stones, but complications are possible.

Q-Do seeds, vegetables or fruits increase the tendency to form gallstones? No. There’s no evidence that eating seeds, vegetables or fruits increases gallstone risk.

Q-Can gallstones be treated with medicine only? Usually no β€” surgery is the most reliable treatment for symptomatic stones. There is a medicine (ursodeoxycholic acid) that can dissolve certain cholesterol stones in select cases, but it’s slow, works only for some stones, and recurrence is common. For most symptomatic or complicated cases, removal of the gallbladder is recommended.

Q-Can surgery be done robotically? Yes. Robotic-assisted cholecystectomy is an advanced form of minimally invasive surgery.

Q-Is complete gallbladder removal mandatory when surgery is done? Yes β€” cholecystectomy (removal of the gallbladder) is the standard operation for symptomatic gallstones and most complications.

Q-Does removing the gallbladder cause long-term problems? For most people, no. Most adapt well to life without a gallbladder. Some patients may have mild changes in bowel habits (post-cholecystectomy diarrhea) or occasional indigestion, but serious long-term problems are uncommon

⚠️ Risk of bowel adhesion (mesh inside abdomen)
⚠️ Use of tackers ,cause of pain
--
eTEP RS πŸ”]
βœ… Mesh placed in retromuscular space
βœ… Great for medium to large hernias
⚠️ More dissection β€” not ideal for very small defects ,need to cut and reconstruct sheath
--
TARM & TARUP πŸ› οΈ]
TARM – similar to eTEP RS but intra-abdominal approach.

***pETEP/TAPP – mesh placed between abdominal wall layers.
βœ… Simple polypropylene mesh – cost-effective
