Fibroids are benign tumours of the uterus that can cause asymptomatic or symptomatic. Symptoms caused by fibroids can be different types of pain, abnormal bleeding such as heavy bleeding, as well as symptoms of bowel and urinary tract. When fibroids put pressure on the bladder it can cause frequent and painful urination. Fibroids can also cause back and leg pain.
In terms of pain fibroids can cause:

A well known symptom of endometriosis is chronic fatigue. Chronic fatigue can cause significant impairment in activity levels, worsening symptoms, sleep problems, as well as difficulties with memory and concentration or orthostatic intolerance.

Some research shows that endometriosis sufferers are twice as likely to have extreme tiredness. Although periods can cause tiredness in general, most endometriosis sufferers feel exceptionally tired, feeling of exhaustion, non-stop nausea, and being sick. They also frequently experience chronic joint and aching muscles.

Fatigue in endometriosis is like a constant exhaustion, feeling tranquilised, unable to keep your eyes open to the point that you feel you can't do anything. Feeling tired or exhausted can also be a sign that a flare-up is on the way.

It is well known that endometriosis impacts all aspects of one’s life. This includes work as well. In two studies, about 40% of endometriosis sufferers report impaired career growth due to endometriosis, and about 50% experience a decreased ability to work due to their chronic disease. This is mainly due to endometriosis-related pain and fatigue, which can make it difficult to meet the demands of a job, affecting the work productivity. Underperformance, as well as frequent sick leave can lead to disciplinary or even dismissal.

A study conducted by researchers from Southern Cross University's National Centre for Naturopathic Medicine and Western Sydney University 's NICM Health Research, on women with a confirmed diagnosis of endometriosis, showed that with 1 in 3 women with endometriosis missed the promotion due to having to manage their endometriosis symptoms, and 1 in 6 were fired due to having to manage their endometriosis symptoms.

Abdominal endometriosis is considered rare and occurs in most cases following a caesarean section. Describedin the 1950s, abdominal endometriosis is classified as secondary endometriosis. There are also cases of abdominal endometriosis without previous surgical intervention, known as primary abdominal endometriosis.

Abdominal endometriosis is frequently found in the cutaneous and subcutaneous adipose tissue at the level of the cesarean scar. Theintramuscular localisation is quite rare. The incidence of abdominal endometriosis in patients with previous gynecological interventions is 0.03-1.08%.

Excision is the only effective treatment in patients with abdominal endometriosis, and complete local excision with clear margins is key to prevent long-term recurrence of symptoms.

Based on the retrograde menstruation theory, endometriosis is considered a disease that can be cured by stopping the menstruation and ovulations. Menopause is often mentioned to patients as a cure. In reality though, menopause has little or no impact on endometriosis.

A team of schools from Switzerland reported a case of endometriosis in a 61 year old patient with a hysterectomy. The said patient had no medical history of endometriosis and she was under hormone replacement therapy.

The patient presented to the hospital for severe metrorrhagia without abdominal pain. A scan showed a heterogeneous highly vascularized lesion above the va**nal dome measuring 53 × 66 mm. The left o***y was not seen. Medical anamnesis revealed that the mass was discovered 5 years before and since all was clear for cancer, it has been left unoperated. Further investigations found a hypodense mass adjoining the va**nal dome possibly infiltrating the posterior bladder wall and the re**um.

What is interesting is that endometriosis was sort of excluded as a possible diagnosis due to the past hysterectomy. The patient undertook a surgery that resulted in a bloc resection of the infiltrated recto-sigmoid with a small part of the va**na, and an ileostomy was performed. The operation lasted 8 h and the initial diagnosis was pointing to a rare form of cancer. The pathological analysis of the large mass that had developed in the sigmoid meso and the muscularis propria concluded to endometriosis and no cancer.

Endometriosis affects 10% of people born with a uterus worldwide. Its a whole body disease that can lead to serious complications if not treated on time. On average it can take up to 10 years or more to receive a diagnosis. Many endometriosis sufferers struggle to do their jobs and in some cases, they are missing out on promotions. Endometriosis impacts fertility as well in many ways.
