Following is the second case in the series of closed NTDs :

28 years G3A2 at 20 weeks 4 day with suspicion of closed neural tube defect at thoraco-lumbar region, referred for second opinion. On scan - normal intracranial anatomy, split cord malformation/ diastematomyelia (with bony spur at T12-L2), hemivertebra at L1, splaying of posterior elements (L1-L5) and tethered cord (till S3) without any cystic lesion. Patient chose to continue the pregnancy post counselling and this patient delivered at term, diastematomyelia was confirmed post delivery with no associated abnormalities. Prenatal images are attached. As per literature, more than 93% of cases have no residual neurological dysfunction post detethering surgery. Dethering is recommended in all cases. Comments are invited.

From suspicion to success! This 38-year-old, 20-week pregnancy with a suspected closed neural tube defect at T5/T6 had us cautiously optimistic. With normal intracranial anatomy and no vertebral malformation, we recommended continuing the pregnancy – and the outcome was beautifully favorable! Post-delivery, the meningocele was successfully removed with no neurological complications. A true testament to careful monitoring and positive results!

We will like to discuss few cases of closed neural tube defects with different presentations and discuss their management. It will be nice to get comments from caregivers in different cases, how they would have counselled the patient in the given setting. Following is the first case in the series - 38 years G4P2L2A1 at 20 weeks 1 day with suspicion of closed neural tube defect, referred for second opinion. On scan - normal intracranial anatomy, small cystic lesion at T5/T6, without any vertebral malformation. We advised continuation of pregnancy and this patient had a favorable outcome. The cystic lesion was confirmed as meningocele and removed surgically post delivery, without any neurological complications. Prenatal and postnatal images are attached.
