Regenerative medicine and Female pattern hair loss

Hair loss in females is complex and multi factorial. Hair transplant is not an option in every single patient except in extreme cases.

We not only have to address nutritional and hormonal factors but local hair follicle microbiome as well.

This case is an illustration how regenerative and micro biome treatments work.

Alopecia Areata once considered most difficult to treat dermatological condition has now multiple treatment options.

2007 when I joined Dermatology, we had an admitted Alopecia Universalis patient, we tried our best to help him out-unfortunately we had limited options or none I can safely say at that time.
2025- We have multiple treatment options- Jak1,2- JAK 1 selective which have got wonderful results

These newer medications have drastically changed the way we look at this disease. These medications are for long term and should be taken only after consulting a qualified Dermatologist. They need baseline investigations and constant monitoring of blood counts, lipids

Many more drugs are in phase 3 trials and future seems quite promising🤲🤲❤️❤️🤝🤝

Number of grafts required to cover frontal area loss is 2500 on an average

If somebody tells you 4000, think twice

If you have very high density in donor and age is on favorable side 3000 grafts can be done. But never beyond.

2500 grafts means around 6000 hair as one graft may have one, two, three or even more hair.

If you color, relax, straight, keratin your hair you could be damaging your hair over time and this can lead to hair loss.

Remedy: Changing the way you care for your hair helps but if harsh chemicals are used repeatedly it may lead to permanent loss of hair.

Hair style pulls on your scalp:

If you constantly wear your hairstyle tightly pulled back it may lead to loss of hair called medically as Traction Alopecia

Remedy: Never should hair be tied too tightly, keep it little loose and choose a style which doesn’t exert pull over scalp.

Chronic and acute Telogen Effluvium

Few months after childbirth, illness, any stressful event or surgery one may find bunches of hair coming out in hair brush/comb. This is because the hair growth arrest occurs during these events.

Once stressful event is over, the body starts to readjust & hair loss stops. With proper treatment and replenishing of lost stores complete hair growth recovery is possible within 6-9 months.

Cause: Disease caused when body’s immune cells attack hair follicles. It can affect any body area-scalp, eyebrows, eyelashes, beard and body.

Is treatment available?

Most of the patients have limited disease and respond to topical treatment-some require Intralesional injections.

Some patients develop extensive disease need long term oral medications

Newer treatment options like JAKi have revolutionized treatment of extensive Alopecia Areata.

1, These therapies are effective in early grades of Alopecia.

3, In higher grades of Alopecia these may be ineffective.

Beard and chest hair for Hair Transplant:

Beard hair is an excellent source of hair to be used for hair restoration after scalp hair. But here are few differences:

1) It’s thicker than scalp hair but mostly exist as single hair which means 2000 grafts from scalp is equal to 3500 grafts from beard as scalp hair exists in 1,2,3,4and5 hair grafts

2) Growing (Anagen) phase of scalp hair is very long as compared to beard hence in beard hair shedding phase occurs more frequently than scalp

3) Beard extraction sites may leave a scar which should be discussed with treating doctor.

4) Chest hair if thick may be used too but 2000scalp grafts=10000 chest grafts.

This reflects the importance of scalp grafts but yes in patients with scarce scalp donor hair beard and chest hair can be used
