Disc Impingement vs GTPS.
Why correct diagnosis matters more than pain location.
Many people believe that all hip or leg pain originates from the spine. In clinical practice, this assumption frequently results in misdiagnosis, delayed recovery, and inappropriate treatment.
Understanding the distinction between these two commonly confused conditions is essential.
1. Disc Impingement (Lumbar Spine–Related Pain)
What it is a Lumbar disc bulge or herniation irritating a nerve root.
Typical features;
●Pain radiating from the lower back into the buttock, thigh, or leg
●Tingling, numbness, burning, or electric-type sensations
●Pain aggravated by sitting, bending, coughing, or prolonged driving
●Possible neurological signs
Clinical perspective:
Disc-related pain usually follows a nerve distribution, not a single localized tender point.
2. GTPS – Greater Trochanteric Pain Syndrome (Hip-Origin Pain).
What it is a local condition involving gluteal tendons and bursae around the outer hip.
Typical features;
●Localized pain over the outer aspect of the hip
●Pain while lying on the affected side
●Discomfort during walking, stair climbing, or single-leg stance
●Pain reproduced on direct palpation over the greater trochanter
Clinical perspective:
GTPS is a mechanical, load-related condition, not a nerve pathology.
Why these conditions are often confused.
Both may:
●Cause buttock or lateral thigh pain.
●Worsen with activity.
●Co-exist in the same patient.
However, treating one as the other commonly leads to incomplete or temporary relief.
Key message for patients
✔ Pain location does not always indicate pain source.
✔ Imaging findings must be clinically correlated.
✔ Detailed assessment is essential
✔ Correct diagnosis leads to faster, safer recovery.
Awareness is the first step to recovery,
●Not all hip pain is a disc problem
●Not all leg pain requires spine-focused treatment
●Assessment before assumption is the foundation of effective care.
