I am an orthopaedic surgeon with more than 11 years of experience in treating bone and joint disorders. This includes over 3500 operations of all degrees of complexity, across the spectrum of orthopaedic surgery. My expertise is in the treatment of complex fractures including pelvic ring and acetabular injuries which are among the most challenging orthopaedic injuries to address. To the best of m

y knowledge, I am the only fellowship trained pelvic/acetabular surgeon in Jammu and Kashmir, having learnt this art from the best in the business at the Royal London Hospital, UK. I also specialise in hip and knee replacements including complex hip replacements done for acetabular fractures, and partial knee replacements. I learnt joint replacement surgery first at Fortis Hospital, Kolkata and subsequently in London at the Royal London Hospital and Barts Health Orthopaedic Centre (BHOC). I practised joint replacement at my previous appointment as consultant surgeon at the Calcutta Medical Research Institute (CMRI). I have special interest in the treatment of bone infections (osteomyelitis), neglected and unhealed fractures (non unions) and children’s orthopaedic disorders. I am passionate about evidence-based practice, patient safety and satisfaction. I believe everyone deserves the best treatment regardless of their geographical location. This is why I gave up my career in the UK, and subsequently my consultant post at the Calcutta Medical Research Institute, to bring back the skills and knowledge here, where I can hopefully make a difference.. Shoulder pain is a common reason for a visit to the doctor. I see my fair share of these too, and I’ve observed that many have been labelled ‘frozen shoulder’ when they’re not. The term is so rampant that many with shoulder pain of any nature just refer to their problem as frozen shoulder at first consultation.
The good news is that most cases of shoulder pain are NOT frozen shoulder. Frozen shoulder is a very specific entity and can be diagnosed only after a thorough clinical examination. The better news is that the treatment of this condition had evolved beyond endless cycles of physio and exercise. Early diagnosis and intervention can cut short the course of this condition and greatly improve pain and quality of life, and also makes exercise much more effective.
Obviously the first step is to make the distinction between frozen shoulder and other more common causes of shoulder pain!

Lower back pain and leg pain from compression of a nerve in the back, whether from a pr*****ed disc or tightness from bony/ligament overgrowth, is one of the most common complaints in my clinic. I continue to be spellbound by how poorly managed this remains. Most people want a quick fix ‘medicine that will make it go away’; my answer is that such a medicine does not exist. Many worry that their nerves are the problem. I reassure that pain from nerve compression does not mean the problem is neurological; it is actually mechanical and when we address the compression the pain goes away. So how do we treat it? >80% get better with back rehab; this includes physiotherapy and a dedicated exercises program. No medicine, no belt, no rest (in longstanding cases); just lifestyle modification and exercises. I get odd looks when I say this; some do not come back. But this is the way. And we will normalise this

Sending photographs of scans or their reports to a doctor via WhatsApp is common. While this is quick and convenient, it is also fraught with risk. Any scan or other test is most useful when the clinical findings are known. This requires the doctor to know the history and have physically examined the patient, to correlate the findings of the scan or look for specific areas that will explain the patient's symptoms. Many scans mention details that may be clinically not significant. Conversely it the scan is self prescribed, the doctor performing the scan may not know what to look for and important findings can be missed. Likewise, reviewing a report without knowing what to look for keeps the doctor guessing, and good medical advice can never come out of it.
One risks missing the diagnosis altogether to over treating for innocuous findings; neither is good.
Desist from this practice. Don't self scan.
