PIC - Poor Quality Day 3 Embryos -
1. Uneven blastomeres
2. Fragmentation +++
3. Fragile zona pellucida

Only 3 poor quality MII oocytes were retrieved in first IVF in a low AMH patient. ICSI was performed causiously to avoid any degeneration or leaking of cytoplasm while ensuring formation of embryos.
On day 3, poor quality cleavage stage embryos were developed and were freezed as development till day 5 did'nt seem to be promising.
FET cycle was planned and all three embryos were transferred.

GOT POSITIVE RESULT WITH 400 BETA VALUE.

Conclusion - Never ever underestimate poor quality oocytes & embryos especially in low AMH patients. As an embryologist, we just need to give our best as we can and be positive always.

Sperm Preparation for IUI.... - The principle is to separate the actively motile sperms from all the other constituents (dead abnormal sperms, debris, pus cells, leukocytes, etc) of the semen. - The post wash (as seen in the video) is concentrated sample of actively motile (capacitated) sperms swimming freely in nutrient rich clean media. - Such sample has very high capacity to fertilize the ova & result in pregnancy.

Sperm Immobilization - One of the critical step for fertilization while performing Intracytoplasmic Sperm Injection (ICSI). - It is important for the rapid release of sperm factors that initiate oocyte activation.

THE STANDARD IVF INSEMINATION TECHNIQUE - First, the oocyte cumulus complex (COC) after maturation period are kept in a culture dish with fertilization media (media rich in its nutrient content as well as all growth factors) - Semen sample is carefully washed with density- gradient technique where after wash we get capacitated (highly progressive motility) morphologically normal sperms separated. - The sperm sample is then inseminated in the culture dish with COC's, cultured together for 16-20 hours. The sperm must penetrate the sperm must penetrate the egg on their own, similar to natural insemination. Pre-requisite for this technique are - good quality oocyte cumulus complex (fluffy white expanded cumulus with visible oocyte) & excellent quality semen report. This techniques avoids the adverse effect of invasive fertilization technique (ICSI) and also it is cost effective.
