A 56-year-old male presented with a persistent cough and breathlessness that had lasted for three months.

A high-resolution computed tomography (HRCT) scan revealed a right hilar bronchial nodule measuring 13 × 12 millimeters. This nodule exerted extrinsic compression on the adjacent right lower lobar bronchus. The peribronchial wall surrounding the nodule appeared “active,” and multiple centrilobular peribronchial nodules (3 to 5 millimeters) were found in the basal segments of the right lower lobe.

During diagnostic bronchoscopy, a fleshy vascular lesion was observed at the origin of the right lower lobar bronchus. There was bleeding on touch, raising suspicion of a possible carcinoid tumor. Consequently, a hot biopsy was attempted.

Surprisingly, histopathology of the biopsy revealed impacted particulates in the bronchus, accompanied by severe inflammation.

Therapeutic bronchoscopy was then performed, and the foreign body was removed in multiple pieces using various therapeutic instruments.

A key lesson learned is that unexpected diagnoses in airway lesions are always possible. It is crucial to have a well-experienced team, including anesthesiologists and skilled pathologists.
