Stages of a Knee Replacement: Yes, in 2025, you can preserve much more of your natural knee than what used to be the scenario 10-15 years back. Our motto has always been minimum invasion, maximum bone preservation and maximum outcomes. Stage 4,5, and 6 are revision surgeries that need to performed in extreme bone loss conditions. To avoid such catastrophes, the first surgery has to be bone preserving. What if the patient has very advanced knee arthritis and really needs a Total Knee Replacement ? Still, if we retain the Posterior Cruciate Ligament, which is the strongest ligament of the knee, we can save 30-40% more bone. Not only that, retaining this ligament has other benefits also, in terms of better stability, more natural movements of the knee, less risk of fall and fractures, as well as increased life of the implants. Robotic assisted Cruciate Retaining Knee Replacements are now the standard of care in US, and Australia.

Steps in a Conventional Knee Surgery: Cut bone, Balance tissues and then Assess. These steps become irreversible. In a Robotic assisted surgery, the steps go absolutely reverse. Assess , Balance (virtually), and then Cut bone. So performing bone cuts come as a last step of the surgery, after the surgery has been perfectly planned. This allows us surgeons to perform fine tuning and achieve perfect balance, and hence a precise surgery.

We saw that it is always wise to preserve bone in the first surgery. The first solution to it is microplasty or partial knee resurfacing, in which we resurface only the inner or medial aspect of the knee. Microplasty saves 75% of our natural knee and 50% patients who have been advised for a Total knee replacement are eligible for a Microplasty.

‘How do you feel now ?’ I ask my patients on follow-up. After Microplasty, most of them reply, ‘I feel normal. મારી સર્જરી થઈ છે એવું મને લાગતું જ નથી.’ Most patients forget that their surgery has been done. Why? Because implants of Microplasty are designed in such a way that they can easily allow natural movements of the knee.
