Continuing with series of lifting lids.

21year old presented with complaints of right eye upper lid droop since birth

On examination she was diagnosed as left eye congenital ptosis and advised surgical LPS plication.

This are results of LPS plication on post op 1 week.

Understanding the pros and cons of any ptosis surgery is a must for a patient and immediate family before undergoing the knife.

Lid twists and turns:

Ectropion is a medical condition in which the lower eyelid turns outward away from the eyeball.

This can cause constant irritation and is more common in old age. In severe ectropion, the entire length of the eyelid is turned out. In less severe ectropion, only one segment of the eyelid sags away from the eye.

Patients often times will complain about the eye not closing completely, excessive watering and an unpleasant appearance.
This patient who also has history of previous ptosis surgery done by me, underwent a lower lid retractor plication with fornix forming sutures to correct the right lower eyelid ectropion and we are happy with the outcome!

Dealing with pain and sleepless nights:

She is a 58 year old lady from Ahwa, Dang who was suffering from watering in both eyes since 3 months and being treated locally with topical antibiotics, since last week she started having pain and swelling which gradually increased and led to inability to open her right eye and for this she was referred to us.

On examination she had right orbital abscess and bilateral dacryocystits which was confirmed in MRI

She although in pain was extremely cooperative and followed up as advised. With sequential right orbital abscess incision and drainage followed by dacryocystorhinostomy followed by left dacryocystorhinostomy. She at her final follow up has completely recovered.

All complaints of watering from eyes needs to be thoroughly evaluated by an ophthalmologist and oculoplasty surgeon to avoid secondary complications, also prompt management is sight and life saving in certain situations.

This patient underwent a lid tightening procedure along with fornix forming sutures to correct the left lower eyelid ectropion and we are happy with the outcome!

Fortunate to be a faculty at Gujarat rheumatology and SGAO collobarative meet

Tiny situation,

This 8 day old girl was referred for examination and management with history of emergency c sec due to iugr and oligoamniosis.

She on examination had upper lid coloboma with congenital symblepheron variant and corneal abscess with exposure keratopathy in both eyes. Further more with low birth weight of only 1800gms anaesthesia was a challenge well managed by Dr Jayal

Post initial examination she was posted for symblepheron release with amg and coloboma closure with canthotomy and cantholysis.
