నమస్కారం!
విరాజ్ గ్యాస్ట్రో అండ్ లివర్ సెంటర్ నుండి మరో challenging case ని మీ ముందుకు తీసుకువస్తున్నాం.

ఇక్కడ మీరు చూస్తున్నది 45yr రోగి ,
ఈయన తరచుగా పంక్రీటైటిస్ (Reccurent pancreatitis) రావడం వలన విపరీతమైన కడుపు నొప్పి తో భాధ పడుతు చాలా సార్లు హాస్పటల్ లో అడ్మిట్ అవ్వాల్సివొచ్చింది.

అల్ట్రాసౌండ్ స్కాన్ పరీక్ష లో పాంక్రీస్ అనే గ్రంధి వాపు వొచ్చినట్టుగా తెలిసింది.
తరచుగా వస్తుండడం వలన workup లో భాగంగా MRI scan చేయడం జరిగింది.
దానిలో ప్యాంక్రియాటిక్ డక్ట్ (Pancreatic duct - అనగా ఈ డక్ట్‌ జీర్ణ రసాలను చిన్న పేగులోకి తీసుకువెళ్తుంది)
లో ఏర్పడిన పెద్ద రాయి (Calculi) ఆ ప్రవాహాన్ని పూర్తిగా అడ్డుకుంటోంది. దీనివల్ల రోగికి తీవ్రమైన కడుపు నొప్పి మరియు ప్యాంక్రియాటైటిస్ (Pancreatitis) వంటి సమస్యలు వస్తాయి.

ఈ రాయిని తొలగించడానికి, మేము ERCP (ఎండోస్కోపిక్ రెట్రోగ్రేడ్ కోలాంజియోపాంక్రియాటోగ్రఫీ) విధానాన్ని ఉపయోగించాము.

ముందుగా, Ampulla ( duct మొదటి భాగం) వద్ద ఉన్న sphincter ను ఒక ప్రత్యేక వైర్ సహాయంతో కొద్దిగా cut చేసి , డక్ట్‌ మొదటి భాగం ను పెద్దది చేస్తున్నాము.

దీనిని ఎండోస్కోపిక్ స్పింక్టెరోటమీ అంటారు. ఇది రాళ్లను బయటకు తీయడానికి మార్గాన్ని సులభతరం చేస్తుంది.
ఇప్పుడు, ప్రత్యేకంగా రూపొందించిన తొలగింపు పరికరం (Extraction Device), ఇక్కడ మీరు చూస్తున్న బల్లన్ (balloon) లాంటి పరికరాన్ని డక్ట్‌లోకి పంపి రాళ్లను జాగ్రత్తగా బయటకు తీయడం జరిగింది

క్లిష్టమైన ఈ ప్రక్రియలో అత్యంత నైపుణ్యం అవసరం.
పెద్ద రాయి లేదా అనేక చిన్న రాళ్లను పూర్తిగా తొలగించిన తర్వాత, ప్యాంక్రియాస్ రసాల ప్రవాహం మళ్లీ సాధారణ స్థితికి వస్తుంది.
తరువాత ప్యాంక్రియాటిక్ డక్ట్‌లో ఒక చిన్న ప్లాస్టిక్ పైపు లాంటి స్టెంట్‌ను (Stent) ఉంచుతాము.
ఈ స్టెంట్ తాత్కాలికంగా డక్ట్‌ను తెరిచి ఉంచుతుంది.
ఈ సంక్లిష్టమైన ERCP విధానం ద్వారా, రోగి తీవ్రమైన నొప్పి నుండి ఉపశమనం పొందారు.

గమనిక : ఇలా Alcohol అలవాటు లేని వారిలో తరచుగా పాంక్రీటైటిస్ వొస్తుంటే , దాకిని గల కారణాన్ని వెతకడం చాలా ముఖ్యం . ఆలా చెయ్యడం వలన పాంక్రీస్ పూర్తిగా పాడవకుండా కాపాడవొచ్చు.
