Spondylosis is a term referring to degenerative osteoarthritis of the joints between the center of the spinal vertebrae and/or neural foramina. If this condition occurs in the zygapophysial joints, it can be considered facet syndrome. If severe, it may cause pressure on nerve roots with subsequent sensory and/or motor disturbances, such as pain, paresthesia, or muscle weakness in the limbs.

Spondylosis is from Ancient Greek spondylos, "a vertebra", in plural "vertebrae - the backbone".

When the space between two adjacent vertebrae narrows, compression of a nerve root emerging from the spinal cord may result in radiculopathy (sensory and motor disturbances, such as severe pain in the neck, shoulder, arm, back, and/or leg, accompanied by muscle weakness). Less commonly, direct pressure on the spinal cord (typically in the cervical spine) may result in myelopathy, characterized by global weakness, gait dysfunction, loss of balance, and loss of bowel and/or bladder control. The patient may experience a phenomenon of shocks (paresthesia) in hands and legs because of nerve compression and lack of blood flow. If vertebrae of the neck are involved it is labelled cervical spondylosis. Lower back spondylosis is labeled lumbar spondylosis.

Aphthous Stomatitis

Description:
- This is one of the most common oral diseases.
- It's a painful lesion that size from less than 1 mm to 2 cm, single or multiple.
- There is three types: The small lesions are less than 0.5 cm and called the "Minor Aphthae", The large lesions are more than 0.5 cm and called the "Major Aphthae", while they may be Multiple and pinpoint seldom exceeding 1 mm and are called "Herpetiform Aphthae".
- The exact incidence is unknown, but estimates range from 20% to 60% of the population.
- The Lesion begins as a red macule, less often papule, but not blisted. Then it ulcerates and the ulcer become covered by pyogenic membrane producing the characteristic yellow-white center with surrounding erythematous flare.
- It's usually round to oval but may be elongated in natural folds such as the vestibule.
- The lips, cheeks, soft palate, floor of mouth, ventral and lateral tongue are often involved but attached gingival, hard palate and dorsal tongue are seldom affected.
(Occurs on freely movable mucosa that does not overlie bone).
- Aphthous lesions affect all age groups from young to old but young adults and females are more affected.
- Also Known as: Canker sores, recurrent aphthous stomatitis, RAS.

Etiology:
- The cause is unknown.
- There are too many theories for them all to be correct. Aphthous stomatitis may not be a single disease with a single cause but instead a variety of diseases all manifested by painful mouth sores.
- The concept that canker sores are caused by a microbiologic agent has been superceded by theories revolving around an immunopathogenesis. The deposition of antibodies and complement within epithelium and basement membrane during the early stages of the disease suggests a humoral immune response, and the influx of lymphocytes rather than neutrophils in early lesions points to a cellular immune reaction as well. It is yet to be learned if the immune response is directed against self (autoimmunity) or against an extrinsic antigen such as bacteria or viruses. To further cloud the issue, a variety of other factors have been implicated. Withdrawal of certain foods such as cheese, tomato products and gluten, as well as sodium lauryl sulfate-containing toothpastes, has been claimed to help some patients whereas in others, correction of iron, B12 and folate deficiencies have brought about a cure. Improvement of aphthous lesions during the last stages of pregnancy with exacerbation after delivery suggests that gonadal hormones may lay a role. The occurrence of canker sores during menstruation also suggests a hormonal basis. To add a final element of mystery, aphthous stomatitis has been reported to worsen when cigarette smoking is discontinued.

Treatment:
- To reduce pain, patients with few lesions may be treated with topical medications such as Orabase® with Benzocaine, Zilactin®, or Soothe-N-Seal®.
- Anti-inflammatory agents such as topical steroids or Aphthasol® have also been shown to be effective.
- For severe or widespread disease, systemic prednisone such as a Medrol 4 mg Dosepak® is helpful.
- Long-term systemic steroid therapy may be associated with numerous adverse effects, including osteoporosis, asceptic necrosis, cataracts, depression, fluid retention and exacerbation of diabetes.

Prognosis:
- Cure is seldom achieved but palliation and long-term remission may be achieved by above mentioned treatment.
- Without treatment, healing time varies from 4 days for a small lesion to a month or more for major aphthae.
- Major aphthae may also cause scarring.

Differential Diagnosis:
- Aphthous stomatitis must be differentiated from herpetic stomatitis, the disease with which it is most often confused.
- Recurrent intraoral herpes occurs almost exclusively on mucosa overlying bone. The hard palate is the most common site.
- Lesions indistinguishable from aphthous stomatitis have been reported in Behcet’s syndrome, Reiter’s syndrome, Crohn’s disease and celiac disease.

Post-Op Instructions After an Oral Surgery
No surgery is ever minor, even surgery of the mouth. Let’s face it – it hurts! But the good news is
that after a little time you’ll be feeling better than you did before.
Of course that’s little consolation in the first couple days, and the below instructions will give you
an idea of what your healing will be like.
Extraction Site
The site of extraction is a large wound in your bone
Avoid eating, drinking and rinsing for three hours. Bite the gauze for the next three hours
for a blood clot to form and stop bleeding. Apply constant pressure to the gauze on top of
the wound.
Don’t touch the wound with your tongue, finger or anything else. Don’t use any kind of
local medications, as they might cause post-inflammation. Don’t suck the wound. Avoid
smoking for at least 24 hours after the surgery.
Clean your mouth after each meal with clean water or a mixture of water and table salt
(apply half a tea spoon of table salt into a glass filled with lukewarm water).
Be careful during the first three days to NOT use too much force when rinsing your
mouth as the blood clot might break and the healing process could be disrupted.
Swelling
It’s perfectly normal for a surgery to cause swelling. Following the surgery, opening your mouth
may be limited by swelling. This is called trismus and will likely go away soon after the swelling
goes away. If it gets worse or you have trouble with breathing call an ambulance.
The range of swelling is determined by individual sensitivity and the operation that was performed.
Swelling is at its peak after two days following the operation and will go away in the next 4 to 7
days. At this point a bruise may occur on the facial area.
The swelling is not harmful, but you may calm it down by setting an ice pack inside of a towel and
set it against the cheek for 30 minutes immediately after the surgery. You can repeat this after one
hour during the first day. Applying cold for too long may slow down the healing process altogether,
so don’t overdo it.
After three days of the surgery you may want to advance the healing process and relieve the pain by
applying a warm compression against the cheek (near the area where surgery was performed).
A good amount of time to use a warm compression is 15 to 30 minutes, 1 to 3 times a day
for 1 to 3 weeks. Don’t overdo it , however, as you may burn your skin.
Constant bleeding can be stopped, most often by using gauze to apply pressure.
Thoroughly wash and clean your hands, then take a piece of gauze or surgical dressing
and roll it up into a small ball the size of your thumb.
Set the gauze ball on top of the wound and bite hard your teeth together or apply a
pressure with a finger for 30-40 minutes without taking the gauze away.
If the bleeding hasn’t stopped in 40 minutes, you can repeat the process mentioned
previously. The most important thing is to follow the instructions carefully and remain
calm in a half-sitting position. Applying cold against the cheek is recommended.
A balanced diet with good amount of proper vitamins, minerals and proteins as well as avoiding too
much of physical stress is the key to a good healing process.
If excessive bleeding, pain, fever, diarrhea, vomiting, rash or other complications should arise,
please contact the dentist treating you immediately, the dentist referring you to a specialist or
another dentist or medical doctor to help you out with the problems.

A mother's dental health impacts not only her oral health but the health of her unborn child's developing teeth .
Increases in gingivitis, plaque, and non-cancerous pregnancy tumors of the mouth happen due to high levels of progesterone and estrogen.Especially conditions get worst in eighth month when progesterone and estrogen levels are high. When hormone levels drop in the ninth month of pregnancy, gum problems start to diminish
Good oral health is important for women of child bearing age because periodontal disease can result in low-birth-weight-babies or pre-term births. So to maintain oral hygiene is essential and should be encouraged.
We encourage pregnant women to have a gum cleaning which is known as scaling during the first trimester and a short check up in each of the following trimesters.Any surgical procedure like dental extraction should be done in second trimester.X-rays must be avoided in the first trimester.
Antibiotics like tetracycline should not be taken by pregnant women,so consulting a doctor for medication is must for pregnant women,as it can cause permanent damage to baby.
Brushing twice with toothpaste which contains trichlosan and gantrez would be advisable.Mouthwash containing 0.12 percent chlorhexidine gluconate will help to reduce gingivitis and short check up at dental clinic in each of the trimesters.
Keep Smiling...

Pay Attention to Life Details to Prevent Mouth Sores Recurrence
How is a Woman’s oral health connected to hormonal changes

Women have special n
eeds at different stages of life, including oral health needs and considerations.

In order to study out whether hormones have a relationship to periodontal disease and specific women’s health issues, a clinical professor reviewed 61 journal articles with nearly 100 studies for a collective answer, and found that female hormones that fluctuate throughout women’s lives can change conditions in the mouth that allow bacteria to grow, enter the blood, and exacerbate certain health issues, such as bone loss, fetal death, preterm births, osteoporosis, temporomandibular disorders, eating disorders and thyroid disease.

Hormonal fluctuations have a surprisingly strong influence on the oral cavity. Puberty, me**es, pregnancy, menopause and the use of contraceptive medications all influence women’s oral health and the way in which a dentist should approach treatment. Women need to be more vigilant about maintaining healthy teeth and gums to prevent or lessen the severity of some of women-specific health issues, although women tend to take better care of their oral health than men
