➡️ What is Pinna ?
The auricle, also known as pinna, is a wrinkly musculocutaneous tissue that is attached to the skull and it functions to capture sound. The auricle is mostly made up of cartilage that is covered with skin. There are two aspects of the auricle: and medial (inner) and lateral (outer). The medial aspect of the ear lobe is attached to the skull and has no major practical significance.

The lateral aspect is concave and presents numerous grooves and ridges. The outer rim of the auricle is called the helix, which then inferiorly ends as soft tissue known as the lobule of auricle (or ear lobe). The helix has three parts: crus, spine, and tail. The crus is the anterosuperior convex arch the helix, the spine the thick superior part of the helix, while the tail is continuous with the lobule. Parallel to the helix is another convex curvature referred to as the antihelix, which has two parts: the triangular fossa bound by the crus of the helix and the antihelix; and the crura of the antihelix which is the widening of the antihelix directed posteriorly toward the helix.

The center of the auricle has a fossa that is continuous with the external acoustic meatus called the co**ha of the auricle. This co**ha is covered with a triangular cartilaginous process called the tragus. The base of the tragus is attached to facial skin, whereas its apex partially covers the entrance to the external acoustic meatus. Opposite to the tragus, there is also a cartilaginous elevation above the lobule called antitragus.

Anamolies of Pinna and external auditory canal can be termed as microtia and congenital aural atresia respectively.
☑️ Microtia: is defined as the abnormal development of the Pinna resulting in a malformed auricle. The deformities caused could range from mild distortion of the anatomic landmarks to the complete absence of auricle.
☑️Congential aural atresia: is defined defined as a failure of development of external auditory canal. Congential aural atresia is always associated with a certain degree of microtia.

➡️ Larynx: A tube-shaped organ in the neck that contains the vocal cords. The larynx is about 5 cm (2 in.) long. It is part of the respiratory system and is located between the pharynx and the trachea. Humans use the larynx to breathe, talk, and swallow. Its outer wall of cartilage forms the area of the front of the neck referred to as the Adam's apple. The vocal cords, two bands of muscle, form a V inside the larynx. Each time a person inhales, air goes into the nose or mouth, then through the larynx, down the trachea, and into the lungs. When a person exhales, the air goes the other way. The vocal cords are relaxed during breathing, and air moves through the space between them without making any sound. The vocal cords tighten up and move closer together for speech. Air from the lungs is forced between them and makes them vibrate, producing the sound of a voice. The openings of the esophagus and the larynx are very close together in the throat. When a person swallows, a flap called the epiglottis moves down over the larynx to keep food out of the windpipe. Also known as voice box.

➡️ What is Vestibular Schwannoma ?
Vestibular schwannomas are also called acoustic neuromas. They are benign brain tumours that start in the nerve that connects the brain to the ear.

Vestibular schwannomas do not spread to other parts of the body. Because they grow slowly over some years, you don’t usually have symptoms for some time.

➡️ When you do have symptoms, they can be:

hearing loss that usually affects one ear
ringing and buzzing sounds in 1 or both ears (tinnitus)
difficulty working out where sounds are coming from
dizziness or vertigo
numbness of the face (this usually only happens in advanced tumours)

➡️ How common it is

Vestibular schwannomas are rare tumours. About 6 out of every 100 brain tumours (6%) are vestibular schwannomas. The average age of diagnosis is around 50 years old.

Rarely, vestibular schwannomas are caused by a genetic condition called neurofibromatosis. People with neurofibromatosis are often diagnosed at a much younger age. They usually develop tumours on both sides of the brain (bilateral).

➡️ Tinnitus is when you experience ringing or other noises in one or both of your ears. The noise you hear when you have tinnitus isn't caused by an external sound, and other people usually can't hear it. Tinnitus is a common problem. It affects about 15% to 20% of people, and is especially common in older adults.

➡️ Tinnitus is usually caused by an underlying condition, such as age-related hearing loss, an ear injury or a problem with the circulatory system. For many people, tinnitus improves with treatment of the underlying cause or with other treatments that reduce or mask the noise, making tinnitus less noticeable.

➡️ Tinnitus is most often described as a ringing in the ears, even though no external sound is present. However, tinnitus can also cause other types of phantom noises in your ears, including:

☑️ Buzzing
☑️ Roaring
☑️ Clicking
☑️ Hissing
☑️ Humming

➡️ Most people who have tinnitus have subjective tinnitus, or tinnitus that only you can hear. The noises of tinnitus may vary in pitch from a low roar to a high squeal, and you may hear it in one or both ears. In some cases, the sound can be so loud it interferes with your ability to concentrate or hear external sound. Tinnitus may be present all the time, or it may come and go.

➡️ In rare cases, tinnitus can occur as a rhythmic pulsing or whooshing sound, often in time with your heartbeat. This is called pulsatile tinnitus. If you have pulsatile tinnitus, your audiologist may be able to hear your tinnitus when he or she does an examination.

Tinnitus affects people differently. For some people, tinnitus can significantly affect quality of life. If you have tinnitus, you may also experience:

➡️ WHAT IS OTOSCLEROSIS ?

An inherited disorder that causes hearing loss due to the ear's inability to amplify sound.
There are three small bones that connect to the ear drum to help amplify sound waves. When someone has this condition, one of the bones is unable to vibrate freely.

➡️ Symptoms may include hearing loss or ringing in the ears. In rare cases, vertigo may occur.
Treatment may include hearing aids or surgery to help the bones of the ear vibrate more freely

➡️ Otosclerosis Risk Factors

☑️ Age: It usually starts when you're young. You can develop otosclerosis between the ages of 10 and 45, but you're most likely to get it during your 20s. Symptoms usually are at their worst in your 30s.
☑️ Genetics: It often runs in families. About half of all people with otosclerosis have a gene that's linked to the condition. But even if you have the gene, you won't necessarily get it.
☑️ Gender: Both men and women get otosclerosis. Women, though, have a higher risk. Experts aren't sure why, but if you're a woman and develop otosclerosis during pregnancy, you're likely to lose your hearing faster than if you were a man or you weren't pregnant.
☑️ Race and Ethnicity: Caucasians are most likely to get it. About 10% develop otosclerosis.
☑️ Medical History: Certain medical problems can raise your chances of otosclerosis. For example, if you had measles at any time, your risk may go up. Stress fractures to the bony tissue around your inner ear also might make it more likely to happen. Presbycusis is the loss of hearing that gradually occurs in most individuals as they grow older. Hearing loss is a common disorder associated with aging. About 30-35 percent of adults age 65 and older have a hearing loss. It is estimated that 40-50 percent of people 75 and older have a hearing loss.

➡️ What causes age-related hearing loss?

There may be many causes for age-related hearing loss. It most often occurs because of changes in the following areas:

1. Within the inner ear (most common)
2. Within the middle ear
3. Along the nerve pathways to the brain

Other things that affect age-related hearing loss:

1. Long-term exposure to loud noise (such as music or work-related noise)
2. Loss of hair cells (sensory receptors in the inner ear) that help you to hear
3. Inherited factors
4. Aging
5. Some health conditions, such as heart disease or diabetes
6. Side effects of some medicines, such as aspirin, chemotherapy medicines, and certain antibiotics
Being white
7. Lower income level
8. Infections
9. Smoking

Each person’s symptoms may vary. Some of the most common symptoms include:

1. Other people’s speech sounds mumbled or slurred

☑️Meniere’s disease is an inner-ear condition that can cause vertigo, a specific type of dizziness in which you feel as though you’re spinning.

☑️It also can cause ringing in your ear (tinnitus), hearing loss that comes and goes, and a feeling of fullness or pressure in your ear. Usually, only one ear is affected. The hearing loss eventually can be permanent.

☑️Meniere’s disease symptoms tend to come on as “episodes” or “attacks.” These symptoms include:

1. vertigo, with attacks lasting anywhere from a few minutes to 24 hours
2. loss of hearing in the affected ear
3. tinnitus, or the sensation of ringing, in the affected ear
4. aural fullness, or the feeling that the ear is full or plugged
5. loss of balance
6. headaches
7. nausea, vomiting, and sweating caused by severe vertigo
☑️Someone with Meniere’s disease will experience at least two to three of the following symptoms at one time:

1. vertigo
2. hearing loss
3. tinnitus
4. aural fullness

Stages
Ménière’s disease develops in two stages. Between these stages, a person might not experience symptoms for extended periods.

Early
In its early stages, Ménière’s disease causes sudden and unpredictable episodes of vertigo.

During these episodes, there will be some loss of hearing, which typically returns to normal once vertigo subsides. The ear may feel uncomfortable and blocked and have a sense of fullness or pressure. Tinnitus is also common in early stage Ménière’s disease.

After a vertigo attack due to Ménière’s disease, a person often has extreme exhaustion and feels the need to sleep for hours.

People may also experience the following during the early stages of the disease:

diarrhea
blurry vision
jerking eye movements
nausea
vomiting
cold sweat
palpitations or a rapid pulse
trembling
Late
Vertigo episodes become less frequent in the late stages of the disease and, in some cases, never come back.

However, balance, hearing, and vision problems can continue. Individuals will feel especially unsteady when it is dark. Hearing and tinnitus usually get steadily worse.

A person might also experience drop attacks. These involve spontaneously losing posture or suddenly falling down while remaining conscious.

☑️Earache: Pain in the ear can have many causes. Some of these are serious, some are not serious.
☑️Otitis media (middle ear inflammation): Inflammation or infection of the middle ear (behind the eardrum). Usually, this is caused by an infection.
☑️Swimmer’s ear (Otitis externa): Inflammation or infection of the outer ear (pinna and ear canal). Sudden cases are usually infections; chronic otitis is often a skin condition (dermatitis).
☑️Meniere’s disease: A condition in which the inner ear on one side malfunctions. Vertigo, tinnitus, hearing loss, and pain are common symptoms.
☑️Tinnitus: Ringing in one or both ears. Usually this is due to damage from noise exposure, or from aging.
☑️Cerumen (ear wax) impaction: Ear wax may block the ear canal and adhere to the eardrum. The eardrum’s reduced vibrations impair hearing.
☑️Ruptured eardrum: Very loud noises, sudden changes in air pressure, infection, or foreign objects can tear the eardrum. The small hole usually heals within a few weeks.
☑️Acoustic neuroma: A noncancerous tumor that grows on the nerve traveling from the ear to the brain.
Hearing loss, vertigo, and tinnitus can be symptoms.
☑️Mastoiditis: Infection of the mastoid bone, just behind the ear. Mastoiditis can result from untreated middle ear infections.
☑️Benign paroxysmal positional vertigo (BPPV): A disruption of function in the inner ear, causing episodes of vertigo. Although not medically serious, its symptoms can be distressing.
☑️Cholesteatoma: This is a benign condition. It is the abnormal buildup of skin within the middle ear and surrounding bones. Often there is a foul smelling discharge associated with hearing loss. The condition usually requires surgery in order to prevent hearing loss.

☑️Ototoxicity is when a person develops hearing or balance problems due to a medicine. This can happen when someone is on a high dose of a drug that treats cancer, infections, or other illnesses.

➡️ What Happens in Ototoxicity?

☑️Ototoxicity damages the inner ear. This part of the ear gets and sends sounds and controls balance. How much damage happens depends on:

1. the type of medicine causing it
2. how much medicine the child got
3. how long the child took the medicine

➡️What Are the Signs & Symptoms of Ototoxicity?

☑️The symptoms of ototoxicity can come on suddenly or show up over time.

➡️Signs of hearing problems include:

1. limited, poor, or no speech
2. not seeming to pay attention
3. problems in school or trouble with learning
4. turning up the volume on the TV or stereo
5. not responding to conversation-level speech or noises as expected.
6. Babies and kids who aren't talking yet won't react to or turn their heads when they hear a loud sound.
7. becoming tired more quickly or more often after hours of careful listening (such as after school)

➡️Signs of balance problems include:

1. falling a lot
2. an unsteady "woozy" feeling that makes it hard to stand up,
3. walk, or climb the stairs without falling
walking with their legs too far apart
not being able to walk without staggering

Certain antibiotics, including aminoglycosides

Quinine-based medications
Other common medications that can cause ototoxicity include the following:

Tricyclic antidepressants
Anti-anxiety medications
