Myth: advanced ILD should not be intubated!
Eye opening case--
61 yrs old male admitted with fever, cough, SOB and recent enteric fever.
CT thorax revealed advanced UIP. Patient became more breathless. Developed septic shock. Intubated ventilated ..lung protective ventilation strategy followed. It was very difficult to ventilate him owing to the fibrosed lung.
ABG : severe type 1 respiratory failure with metabolic acidosis.
Prone ventilation done.
Antibiotics adjusted accordingly.
Had a very stormy course in the RITU stay.
Finally we were able to extubate him and finally he going to his "home sweet home".
It's a pleasure to intensivist when the patient get discharge.
Thanks to the RITU team of doctor and nurses.

Young women admitted with complicated dengue shock syndrome, Septic shock, Multi organ dysfunction, repeated weaning failure. Needed tracheostomy. Developed multiple site sepsis.
With all the hindrances overcome, she is now getting discharged to home.
