Angina is chest pain or discomfort that occurs when your heart muscle does not get enough blood. Angina may feel like pressure or a squeezing pain in your chest. The pain may also occur in your shoulders, arms, neck, jaw, or back. It may also feel like indigestion.

Angina is a symptom of coronary artery disease (CAD), the most common type of heart disease. CAD occurs when plaque builds up in the coronary arteries. This buildup of plaque is called atherosclerosis. As plaque builds up, the coronary arteries become narrow and stiff. Blood flow to the heart is reduced. This decreases the oxygen supply to the heart muscle.

Types of Angina
Angina is primarily classified as stable angina and unstable angina.

Stable angina
Also known as effort angina, this refers to the more common understanding of angina related to myocardial ischemia. Typical presentations of stable angina is that of chest discomfort and associated symptoms precipitated by some activity (running, walking, etc.) with minimal or non-existent symptoms at rest or with administration of sublingual nitroglycerin. Symptoms typically abate several minutes following cessation of precipitating activities and recur when activity resumes.

Unstable angina
Unstable angina is defined as angina pectoris that changes or worsens even at rest.

It has at least one of these three features:

it occurs at rest (or with minimal exertion), usually lasting >10 min;
it is severe and of new onset (i.e., within the prior 4–6 weeks); and/or
it occurs with a crescendo pattern (i.e., distinctly more severe, prolonged, or frequent than before).
Unstable angina may occur unpredictably at rest which may be a serious indicator of an impending heart attack.

Angina Treatment
Angina could indicate a medical emergency and a trained medical professional should be consulted as soon as possible if you suspect you have angina.

What is a**l fistula
Fistula means an abnormal connection or passageway between organs that do not normally connect.
Most commonly it occurs near the a**s. This condition is termed as fistula-in-ano. It occurs when a boil or an abscess develops in a**l glands. If the opening of these glands get blocked then the infection gets confined to that area as it is unable to drain out into the re**um. It balloons up to form a pus boil or an abscess and it will keep growing inwards to form a tract. This tract is called fistula whose primary opening in inside the a**l ca**l and secondary opeining is in the peria**l skin.
Every time the opening gets blocked an abscess will develop and the tract will increase in size. Sometimes in a chronic case we may see more than one opening and multiple tracts.
It can happen in 10 out of 100,000 population. It is more common in males. It is usually common in people aged between 30 and 50 years.

Ischaemic Heart Disease (coronary artery disease or CAD) is a condition in which atheroma (fatty deposits) builds up in the linings of the walls of the coronary arteries. This causes a narrow artery and reduced blood flow to the heart muscle. This process can occur in other arteries of the body. Important arteries which, if affected can have medical consequences, are the carotid arteries in the neck, (which supply blood to the brain), and the femoral/popliteal arteries in the legs. In these situations a bruit (noise) may be heard over the area affected (i.e. carotid or femoral arteries), through a stethoscope.

There are many myths in low back pain remedy regarding diagnosis and treatment among the general population. Let us see what are these myths and what are facts.

Myth-1 Forward bending is very harmful in back pain

Fact: In certain conditions like facet arthropathy, spinal ca**l stenosis etc forward bending reliefs symptoms and not harmful. As a general rule, lifting something bending forward should be avoided particularly in slip disc, but slight forward bending that is needed in our daily life living is not prohibited.

Myth-2 Lying down on a hard surface is needed for recovery from back pain.

Fact: Sleeping on hard surface may produce more back pain. It may also cause disturbance in sleep which is again harmful. Mattress should not be too soft. Patient should be allowed to continue to sleep at bed that is comfortable.

Myth-3 Spondylosis is the most common cause of the low back pain.

Fact- Spondylosis is a not a clinical but radiologic terminology and it merely indicates degenerative change in any part of the spine. It does not constitute a specific diagnosis. The most common cause of low back pain in young and middle aged population is disc degeneration and in elderly age group it is facet joint arthropathy,

Myth-4 Complete bed rest, is needed when someone have acute low back pain

Fact- Bed rest of more than 3 days is harmful and restriction of movements is not indicated. Person should be allowed to move within limitation of pain. Strict bed rest causes muscle atrophy, lung complications, deep vein thrombosis, osteoporosis and metabolic disorders. Complete bed rest must be avoided.

Myth-5 Lumbo-sacral belt or brace helps in reducing the pain.

Fact- Lumbo-sacral belts or braces does more harm in chronic back pain. Prolonged uses causes muscle atrophy. Belts and braces must be avoided in chronic mechanical low back pain.

Exposure to excessive heat can cause loss of body fluids and a rise in body temperature. This may occur at any age but is most common in babies and elderly people. It can occur due to prolonged exposure to high atmospheric temperature or a brief exposure to very high atmospheric temperature (direct exposure to sun is not necessary) or due to heavy manual work in high temperature, high humidity. Pre-existing chronic diseases and alcoholism are some of the predisposing factors.
In a hot environment, the body loses heat by diverting blood to the skin and by sweating. Profuse sweating may lead to an excessive loss of fluids and salts, resulting in heat exhaustion. This condition is rarely serious but if exposure to heat continues, the body’s normal cooling mechanism breaks down and the temperature of the body rises further, resulting in a heat stroke. Heat stroke can prove to be a life-threatening emergency.
Heat exhaustion and heat stroke most commonly occur above 104 degree F (40 degree C). High humidity increases the risk of heat stroke because sweating is ineffective and heat loss is decreased.
Heat Cramps
Heat cramps, also known as stoker’s cramps and fireman’s cramps, are painful involuntary contractions of large voluntary muscles of the arms, legs and trunk. They appear in men who work in hot, humid places, lose much water by copious sweating (normal response of body to environment) and drink water to replace that lost in perspiration. These patients are NOT dehydrated but have electrolyte deficiency.
Oral fluids and electrolyte replacement will be sufficient.
Heat Exhaustion
This is an abnormal condition marked by weakness, dizziness, nausea, muscle cramps, and fainting. It is caused by low levels of body fluid and salts resulting from exposure to intense heat or the inability to adjust to heat. Body temperature is near normal; blood pressure may drop but usually returns to normal as the person is placed in a lying-down position; the skin is cool, damp, and pale. The patient IS dehydrated. The person usually recovers with rest and replacement of water and salt.
Heat Stroke
A severe and sometimes fatal condition that results from the failure of the body to regulate its temperature. This is caused by prolonged exposure to the sun or to high temperatures. Lessening or lack of sweating is an early symptom. Body temperature of 105 degree Fahrenheit or higher, fast pulse rate, hot and dry skin, headache, confusion, blackouts, and convulsions may occur. The patient may have fluid and body sodium depletion as seen in classic heat exhaustion.
WHO IS AT RISK?
Heat exhaustion and heat stroke may affect otherwise healthy people, particularly after physical exertion in a hot climate. People who come from temperate climates and travel to the tropics need time to acclimatize to the heat before they can safely exert themselves.
The body’s cooling mechanisms are less efficient in babies and elderly people, making them more susceptible to heat exhaustion and heat stroke. Obesity, diabetes mellitus, alcohol dependence and chronic heart failure, all decrease the body’s ability to lose heat. Diarrhea may contribute to dehydration and increase the risk of developing heat exhaustion and heat stroke.
WHAT ARE THE SYMPTOMS?
After prolonged exposure to hot conditions, the following symptoms of heat exhaustion may develop:
1. Profuse Sweating
2. Fatigue
3. Muscle cramps
4. Nausea and vomiting
5. Faintness and unsteadiness
6. Headache
If exposure to heat continues, the body temperature rises and heatstroke may develop, causing symptoms such as:
1. Cessation of sweating
2. Body temperature of 105 degree Fahrenheit or higher
3. Rapid and shallow breathing
4. Rapid heartbeat
5. Elevated or lowered blood pressure
6. Confusion and disorientation
7. Seizure
8. Fainting, which may be the first sign in older adults
Left untreated, heat stroke may progress to coma. Death may result due to kidney failure, acute heart failure, or direct heat induced damage to the brain.

Claustrophobia

Claustrophobia is the fear of enclosed places. It is fear of not having an easy escape route and often leads to panic attacks.

Causes

- Traumatic childhood experience (such as being trapped in a small space during a childhood game)

- Unpleasant experiences(such as being stuck in an elevator)

Symptoms

- Fear of restriction

- Fear of suffocation

- Removes clothing during attacks

- Sweating

- Increased heartbeat

- Nausea

- Fainting

- Light-headedness

- Shaking

- Hyperventilation

- A fear of actual imminent physical harm

Claustrophobics dislikes small rooms,locked rooms,cars, tunnels, cellars, elevators, subway trains, aeroplanes, caves, crowded areas,sitting in a barbers chair, waiting in line at a grocery store and undergoing an MRI or a cat scan,simply out of a fear of confinement to a single space.
