Sharing a case of orthognathic surgery....Skeletal Class III with mandibular deviation to right side leading to facial asymmetry....Management done orthodontic decompensation followed by Bijaw Surgery.....maxillary advancement with Lefort I osteotomy and mandibular setback with Bilateral Saggital Split osteotomy followed by postsurgical orthodontic corrections.....Highly obliged to my friend and Consultant Orthodontist Dr Vipul Sharma for providing his valuable support & guidance throughout....in this journey of 12-18 months......

....Nasal Chopping was a common punishment in ancient era.....even in Ramayana....Laxmana chopped the nose of Suprnanakha....sister of Ravana.......Sushruta the father of Surgery who hails to Kashi (Varanasi)........performed his first surgery ....in which he performed the median forehead flap for nose reconstruction........Today I am sharing a similar case ....in which the right side alae of nose was chopped due to local fight between the villagers.....The patient reported in our Emergency...with this condition.....The reconstruction was done and the acceptable result was obtained......Now my question is to my friends....can you tell me what was the mode of reconstruction????

.....Sharing a case of young teenager girl with Central Giant cell tumor of Anterior region of Mandible.....Multiple conservative procedures done previously but failed and extended to present size.....Treatment......En bloc resection with adequate margins and reconstruction with Vascularised free fibula flap .....Fibula bone is considered as gold standard for mandibular reconstruction.... postoperative results are amazing with proper cosmesis and achieving the goal......Next phase is rehabilitation with implant supported fixed dentition.......planned after 3 months

Case of the day...... Submental flap in Oral reconstruction
Squamous cell carcinoma of alveolus, Mandible ( Oral Cancer )......T3N0M0....Stage III disease..... treatment .....wide local excision...with segmental mandibulectomy....Supraomohyoid neck dissection followed by application of recon plate and Submental flap approximately 6x4 cm for intraoral defect......it's a good flap with minimal morbidity and good reliability....no facial scar ( Nasolabial Flap)....no shoulder morbidity ( PMMC flap)......Single incision for both primary as well as flap harvest .....primary closure with no need of skin grafting.........overall amazing flap for N0 disease ...
